Adjusting and cleaning of a tail pulley on a 110' Masaba Conveyor LOTO was not performed by the EE before entering the tail pulley section of the conveyor. Belt was energized by the operator; EE was in tangled in the pulley from his feet to his mid section of his body. EE was fatally injured.

Employee was installing new drive belts on a conveyor sheave. As employee was rolling the new belt on the sheave their right thumb was caught between sheave edge and belt causing a laceration to thumb.

Employee was attempting to remove a nut from a screen deck bolt. Bar that employee was prying with slipped and employee cut left hand ring finger.

TWO EMPLOYEES WERE MOVING A SCREEN WHEN IT SLIPPED OUT OF ONE'S HAND CUTTING HIS FINGER. HE NEEDED STITCHES ON HIS MIDDLE FINGER.
