Caught in, under or between a moving and a stationary object

Employee was hitting the clogged chute and pinched his ring finger between the moving sledge hammer handle and the vibrator. He felt the impact but thought it insignificant. A few minutes later he noticed some blood on his finger. No problems with the injury until 12/28/10.

Over-exertion in lifting objects

Alledgedly hurt his back while loading 10/4 power cable into a p/u truck on 10/21,regularly off 10/22-24,worked 10/25,reported accident 10/26,worked 10/27,10/28 went to workers comp Dr.and was wrote out of work.First day missed 11/1/2010.Witness did not see him get hurt or show signs or was not told that he hurt himself. 11/1 mine idled workers transfered to another location.

Over-exertion (Not Elsewhere Classified)

Employee had removed a plate from the chip chute and was clearing the build-up from the chute when his jacket became entangled in a rotating shaft. He did NOT get pulled into the shaft. His injury was caused from the tearing of the jacket.

Fall to the walkway or working surface

The employee was involved in snow clean up at the rotary kiln tunnel. We had 24 inches on the ground and it was still snowing. The loader cleared the area which compacted the remaining snow on the ground and made the area slick. The employee slipped on the snow and struck his left knee on the ground. Employee sustained a knee sprain.
