Fall from scaffolds, walkways, platforms

Injured was standing on a platform 4.5 feet off the floor, attempting to attach a come-along when he lost his balance and fell to the floor.

Caught in, under or between a moving and a stationary object

Injured was helping to install a new 30"" conveyor belt at the HiCal crushing plant. Injured had put clamps on the belt in order to pull the belts together when one side hung up on part of the frame of the conveyor. Injured lifted up on the chain that was hooked to the clamp and pinched his right hand between the clamp and the angle on the conveyor.

EE was wearing all PPE, when flipping welding hood down particle went into right eye. Could not was particle out with eye wash. EE went to urgent care .
