Fall to the walkway or working surface

Employee fell at work bruising his right knee on 4/14/14. On 5/29/14 employee went to the doctor to find that cellulites had set in his right leg. Employee was prescribed antibiotics to treat the illness.

Struck by falling object

Employee was removing a rock from the walkway of loaded barge. Employee reached down with his left hand to slide the rock off when another rock that was resting on top of the rock he was trying to move released and came down on his left index finger.

Fall from ladders

It was partly cloudy. 50 degrees. Employee was wearing rubber boots.

Struck by falling object

Employee was cutting the tabs off of Polydeck screens. Employee went to move a ratchet form that was sitting on the stack of Polydeck panel. The knife slipped out of his right hand and cut his left thumb.
