Employee was accessing the second deck of a Diester screen to check screens and their knee dislocated while getting in the screen. After imaging it was determined they had torn ligaments. Employee is now scheduled for surgery on 4/10/26

Over-exertion in wielding or throwing objects

Employee was shoveling frozen material from under a conveyor belt. Employee went home and their back started hurting. Employee had a strained lower back muscle and was prescribed a steroid pack and muscle relaxer.

Employee was removing snow and ice from a conveyor belt with a flat nosed shovel. While pushing the ice off the belt, the shovel caught on the belt and employee's body weight went forward. The wooden handle came back and hit employee in the mouth. This caused 2 teeth to break and had to be extracted. Antibiotic prescribed.

Visitor incident report. Visitor was walking when visitor tripped and hit head on the ground. Visitor was transported to the local hospital where visitor received stitches.

Struck by flying object

Employee was removing a side wall bucket guard on a loader. The guard was stuck. EE used a 4lb hammer to dislodge it. When EE hit it a piece of metal chipped off and went into EE's leg. Removed in ER. Returned to work, no stitches, antibiotic prescribed.

Struck by flying object

Dust got in the eye of the employee when they went to retrieve a wrench they dropped. The wrench fell onto a covered belt. Employee hit the cover and dust got into their face. No foreign bodies were found when seen at the doctor. Antibiotic drops were prescribed as precaution.

Rubbed or abraded

Deckhand was moving a cable when a burr went through their leather glove and punctured left pinky. The burr had to be removed at the hospital. Antibiotic was prescribed and tetanus shot given. Returned back to work. No other treatment given.

Employee was up on Pc650 Excavator fueling it. EE missed a step with their foot and twisted leg and fell still on the excavator. Two workers had to help get EE off the excavator. EE injured left leg. Extent of injuries are unknown at this point.

Employee was picking up a 50 gal drum, used as a trash can, and dumping it into a dumpster. When EE went to lift the drum, EE hurt left arm. After medical evaluation EE had a torn left bicep tendon that required surgery to repair.

Employee was removing a pin on a cylinder with a sledgehammer and grizzly bolt. Employee swung and missed the bolt used to drive the cylinder out and smashed right index finger requiring stitches.

Day Shift Quarry Manager was inspecting a self retracting life line when they experienced shoulder pain. After a period of diagnostic imaging, it was determined that they had a torn tendon in their shoulder. Surgery was needed to repair the tendon. Surgery was completed on 8/30/19.

QC Tech noticed no rock coming off the belts at the Wash Plant. Went to investigate and noticed the plant operator trying to clean a chute that had backed up. The rod the operator was using to clean the chute out came out and hit the QC Tech cutting right ear.

Employee was helping install a cutting edge on the motor grader. EE right hand index finger and ring finger was pinched between the cutting edge and the mold board. Stitches were required.

Employee was using a pocket knife with EE's left hand to cut a piece of plastic. The knife slipped and cut EE's right index finger.

Struck by falling object

Employee was removing a rock from the walkway of loaded barge. Employee reached down with his left hand to slide the rock off when another rock that was resting on top of the rock he was trying to move released and came down on his left index finger.

Employee fell at work bruising his right knee on 4/14/14. On 5/29/14 employee went to the doctor to find that cellulites had set in his right leg. Employee was prescribed antibiotics to treat the illness.

Struck by falling object

Employee was cutting the tabs off of Polydeck screens. Employee went to move a ratchet form that was sitting on the stack of Polydeck panel. The knife slipped out of his right hand and cut his left thumb.

It was partly cloudy. 50 degrees. Employee was wearing rubber boots.

Miner was hooking up a trailer to his pickup. Miner felt no pain in his right arm while lowering the jack. Miner returned the trailer and unhooked the trailer. Miner felt no pain while unhooking the trailer. While in office miner felt tenderness in his right arm.

Fall onto or against objects

Employee was descending from the top of a metal conex, after securing the conex for transportation. Employee fell while descending, landing on the bed of the trailer.

It was a cool sunny morning. Employee was walking up the stairs to operate plant when his knee popped.

Deckhand was washing the M/V Super Stone when he stood up and hit his head on the A/C unit on the deck.

EE was walking from a loaded river barge to the stationary barge. EE slipped and fell on his left leg. EE sustained a broken fibula and tibia.

Employee engaged a limit switch on a skid steer activating the bucket. The door of the skid steer was open and employee's finger was caught between the door and the bucket.

Fall from machine

Fell while cleaning a conveyer belt while using a hose to clean off the equipment. An employee had fed too much hose to claimant which threw him off balance. Claimant hit his head even though he had hard hat on.

Employee was climbing off a piece of equipment and fell about 3 feet. He caught himself with his right arm and sprained his right thumb.

Employee stated that as he was walking back to the plant, his ankle was sore and hard to walk on.

Cleaning up around Jaw Crusher, where they use a fire hose to wash out from under the jaw. It cut a wash spot out and the employee stepped in the wash out spot about 4"" deep, in which the employee twisted his left anke.
