Contact with hot objects or substances

02/28/13 EE using cutting torch for maintenance. EE claimed slag fall in left ear. EE taken ER,examined,given antibiotic,released w/o restriction. 09/11/2013 EE contacted mgmt via phone complaining of drainage/dizziness. 10/04/13 EE saw ENT specialist. Examined, given antibiotic. Testing revealed perforated ear drum. 01/07/2014 EE underwent surgical procedure to repair ear drum.

Struck against stationary object

Miner was shoveling spilled material inside the BC-01 tunnel. As he moved from one side of the belt to another, he stepped on loose rock spraining his left ankle.

Struck by... (Not Elsewhere Classified)

EE was installing belt wiper on BC02 to divert stone. EE used his personal pocket knife to ""mark"" the belting in order to punch holes in the belting for mounting. While EE was ""marking"" the belt, his knife closed, causing a laceration to his right ring finger. EE was seen at local hospital, required 6 stitches to close wound, and released without restrictions.
