Struck against stationary object

Miner was placing tools and materials in the back of a maintenance truck when EE felt something in EE's boot. When EE took another step a nail protruded through EE's boot sole and into EE's right foot.

Fall to the walkway or working surface

Employee claims left leg slipped behind them while exiting the restroom. Employee claims left knee made contact with the floor. Employee stated that there were no physical hazards present. Employee did not seek medical attention until 12/24/18, and provided a work restriction form on 12/26/2018.

Struck against stationary object

Miner was working to change the table in a VSI Crusher. Miner strained left ankle when EE stepped on a 2"" stone on the catwalk of the VSI Deck.

Over-exertion in lifting objects

Miner reported 03/15/2018 that they had sustained a work related injury to the back. The miner was moving/lifting a 4"" rubber flex hose for the dust conveyance system in order to free a clog on the morning of 03/14/2018, when they noticed they were experiencing back pain.

Struck by falling object

EE was trying to put the crane pad back on the crane deck. When EE threw pad up on the crane deck, the pad landed on their left middle finger resulting in a laceration.

Fall to the walkway or working surface

Miner was attempting to pull a piece of conveyor skirting from a chute when EE lost balance and fell backwards onto the catwalk. Miner sustained a lacerations to EE's left hand requiring 4 sutures to close.
