Employee was removing a roof bolter tire from the rim and while striking the rim with a sledge hammer, the hammer came back and struck employee on employee's left arm.

Employee was operating a scoop and ran over a rock. This caused the employee to be jarred inside the operating deck of the scoop.

Employee was unloading timbers and one of the timbers fell on employee's foot.
