Caught in, under or between a moving and a stationary object

Employee was lying on employee's side changing a torque shaft on the continuous miner when a large rock slid off the miner and pinned employee between rock and frame of the miner.

Employee was on EE's knees operating the continuous miner when a piece of draw rock measuring 2'x2'x2"" fell striking employee on EE's left foot

Fall onto or against objects

When employee released the brake on the rail mantrip, the mantrip began to roll forward. Employee panicked and jumped off. EE slipped and struck EE's left shoulder on rib/brow.

Struck by flying object

Employee was operating a shuttle car. While being loaded, a rock fowled in the discharge end of the continuous miner conveyor chain. When the rock came loose, it was thrown toward employee, striking them on head behind right ear.

Employee was operating a shuttle car when a piece of rock was thrown from the continuous miner boom and struck employee on face and forehead.

Accident type, without injuries

A roof fall measuring approximately 55 inches high, 20 feet wide and 40 feet long, fell in crosscut between no. 4 and no. 5 entry near spad no. 2001. (The fall has been dangered off and will not be cleaned up)

Struck by flying object

Employee was loading roof bolt supplies onto the roof bolter when a co-worker threw a bundle of roof bolt plates and caught employee's finger between the plates and the roof bolter.

Employee stated that while walking in the No. 4 entry, EE slipped and fell causing EE to strike EE's shoulder on a rock. Employee did not return to work after scheduled 2 week vacation.

Accident type, without injuries

A roof fall occurred while mining the No. 1 lift off the right wing of the No. 6 Pillar block on the mains section. The material caught the continuous miner on the left side and caused the continuous miner to be stuck. The continuous miner was pulled out from under the rock by using a separate machine.

Employee was shoveling coal onto the belt conveyor when EE felt a pain in EE's left shoulder.

Employee was operating a shuttle car and was being loaded by the continuous miner when a piece of rock being discharged by the miner struck the employee on EE's lip.

Employee was using a hammer to remove a bearing from a shaft, when EE struck the bearing, a piece of metal flew back and struck EE on the left eye (Employee was wearing safety glasses at the time).

Employee was tramming the roof bolter past a parked shuttle car. The bolter slid and caught employee's leg between the bolter and the shuttle car bumper.
