Caught in, under or between a moving and a stationary object

Craftsman was working to replace a wear plate on a shovel dipper door when the cable supporting the plate caught on the door creating stored energy in the system. The craftsman tried to free the plate with a pry bar and as the cable suddenly released; the craftsman sustained a crushing injury to his middle and ring finger(s), by contact with the wear plate and dipper door.

Employee was performing a pre-operational inspection of his haul truck when he lost balance injuring his left knee resulting in a meniscus tear.

Employee was disassembling the casing cutter. While removing the vertical slide from inside the housing of the casing cutter, there was stored energy that caused the vertical slide to eject from the housing striking the employee in the chest.

Contractor employee had just completed the use of a knuckle-boom crane when the outrigger deployed striking the top of his right foot.

Employee trips on uneven ground causing drill casing to fall on the employee's thigh resulting in a fracture to the left leg.

Employees were moving an air compressor cooling unit with a forklift. While setting the unit down it fell over striking an employee causing a fracture of his right ankle which required surgical repair.

The employee had thought that the survey tool was falling and went to grab it when it hit the ground smashing his left hand between the survey tool and the ground.

While backing up the Forklift the employee in the man basket was thrown up off the deck, the fall protection stopped the employee brining him back down and the employee lacerated his ear and chin on the man basket hand rail.

Employee was tightening bussing from transformer to gear. Wrench slipped off striking #8 tooth. Employee went to see dentist. Dentist wanted to wait to see if pain subsided. No prescription meds, root & cap placed on Friday Dec. 5th became reportable.

A welder craftsman sustained a laceration to his right cheek from 6"" angle grinder equipped with a cutting wheel.

Employee was assisting in moving 1/2"" steel plate when his right ring finger was pinched between the plate and his boot toe causing a crushing injury. X-Rays were positive for fracture of the end bone and sutures were required to close the wound.

AN EMPLOYEE WAS REPOSITIONING A GET (GROUND ENGAGING TOOL) TOOTH IN A SERVICE TRUCK WHEN A SECOND TOOTH SHIFTED AND PINCHED THE EMPLOYEES FINGER BETWEEN THE TEETH.

A worker was struck on his lower lip by the pry bar tool he was using to release a lever binder. The impact caused a 1/2"" laceration to his lower lip and the loss of one of his lower incisor teeth. He reported that the release of energy was more powerful than he had anticipated and he could not keep the bar from striking his face.

Fall from machine

Miner was descending haul truck ladder and fell to ground injuring his ankle. Initial X-Ray was Negative. Miner is no longer employed by the Company. Injury remained painful and was re-X-Rayed finding a fracture. Insurance company notified of positive X-Ray on 3/11/14 and surgical repair on 3/13/14.

Employee was helping to strip coating from electrical cable when his index finger contacted the blade of the stripping tool resulting in a laceration injury.

Employee was hooking up tongs to break 9"" collar and twisted to lock the jaws after locking employee stood up and felt pain in lower back.

Absorption of radiations, caustics, toxic and noxious substances

Mechanic was reconnecting 2 twelve volt batteries when wrench accidentally contacted the battery hold down causing an unexpected sudden release of battery acid.
