Struck by falling object

EE resealing track adjuster lock cover on CAT D11T unit #1601, as the access cover was removed oil leaked out, the EE was attempting to wipe up oil with one hand and hold the cover with the other. Cover slipped from his hand, as he grabbed it the cover fell to the ground smashing his left hand between it and the concrete floor. Per phone call ring finger amputated at 2nd joint.

Fall to the walkway or working surface

The employee was walking on grating at a pump station, employee slipped and fell on some ice. This resulted in employee striking right knee on the grating. Employee had knee swelling and complications weeks later......(Through medical evaluations, it was determined to be work related on 5/20/2016).

Fall onto or against objects

As a driver exited his vehicle, he slipped on ice, twisted his knee and fell hitting his back and head on the running board of the vehicle. After evaluation at a local hospital, he was diagnosed with a sprained/swollen knee and strained back.

Absorption of radiations, caustics, toxic and noxious substances

An overcharged product delivery hose containing compressed air and lime pebbles separated from the connective fitting. The resulting blast of air and pebbles sprayed the driver's torso. The lime pebbles penetrated the driver's skin and burned him.

Contact with hot objects or substances

Employee received 1st Degree burns on his Penis while air arcing on an excavator bucket. Initially the employee did not require medical treatment. Due to the location of the burn, medical treatment became necessary as the wound scabbed over near the urethra. Employee received medical treatment on 12/21/15.

Over-exertion in pulling or pushing objects

Employee was opening a gate valve on water when he felt a pain in his left elbow. The pain did not completely go away and the employee was evaluated by a physician on 9/15/2015. The employee was diagnosed with a tear of the left bicep tendon. The condition will require surgical repair.

Caught in, under or between a moving and a stationary object

Employee was trying to slow down the spool on the fiberglass survey tool to keep it from over spooling and his thumb was pinched in between the spoke of the reel and the beam on the rig.

Absorption of radiations, caustics, toxic and noxious substances

Employee was adjusting shims on dipper of shovel when grease fell into his hair. Employee went to change house facilities and attempted to wash out grease with cleaning fluid simple green, and then used Stride Citrus SC an industrial cleaner used by the janitors. A mixture of these cleaners found a path to the employees eyes during rinsing.

Caught in, under or between a moving and a stationary object

Craftsman was removing wheel motor thrust washer inspection cover. As he removed it, it slipped and he attempted to catch it and his right index finger was pinched between the cover and wheel motor requiring eight stitches and splint.

Struck by flying object

Haul truck operator reported eye irritation in his left eye. Evaluated by EMT and sent to the KUC clinic where a small metallic object was removed from the middle of his cornea. The operator was given some wetting drops for lubrication and returned to work.

Fall to the walkway or working surface

Employee fainted while walking, striking his face, resulting in a fracture injury to his nose and a laceration requiring sutures in his upper lip.

Struck against a moving object

The dozer operator working the Code 25 dump observed a crack. Simultaneously, the operator of haul truck 421 arrived and positioned parallel to the dump approximately 75 feet away from the crest. As the truck operator initiated dumping, another crack propagated, the dump face failed, causing the truck to descend with the dump. The haul truck operator sustained minor injury.

Operator was attempting to clean the window of his dozer when he slipped on the decking, fracturing his cheek bone.

Struck against stationary object

Employee was cleaning the inside of his operator cab when he contacted a sharp edge resulting in a laceration to his right ring finger requiring sutures.

The death was caused by a personal medical condition. The employee was found unconscious and non-responsive. The incident is currently being investigated by MSHA as a non-chargeable incident.

The employee became confused and said that he wanted to find his father. He also claims to hear voices.
