Struck by falling object

Employee was pushing a roof bolt when a rock came down and struck wrist, resulting in a laceration on left wrist.

Fall onto or against objects

Replacing conveyor belt on number four conveyor in the equipment slope. Employee tripped and fell into the conveyor frame causing injury to right eyebrow. (Cut 10 stitches)
