Employee ran equipment while it was overheating.

Injured worker was driving down into the mine shaft and the brakes went out in the truck EE was driving. EE hit a wall to avoid hitting the vehicle in front of EE and the truck flipped upside down.

Employee was cleaning chute when pressure washer hose got loose and smashed employee's finger against the rail, causing a laceration
