Caught in, under or between collapsing material or buildings

A hillside collapse due to a geological anomaly. The equipment operator had made approximately 10 passes at the new cut. As the equipment operator was backing up to make another pass/cut, the hillside collapsed. The force pushed in the window and partially filled the cab of the dozer.

Struck against a moving object

Employee stated they were exiting the pit in a haul truck with a full load. They further stated they had their foot sideways on the ""lock-up"" button and hit a bump which caused them to move up and down on the air-ride seat causing them to feel pain in their left knee.
