Struck by powered moving object

Laborer was moving portable compactor from top of access road to bottom of access road. Compactor was in front bucket of backhoe. As laborer came to a stop, backhoe tires slopped and slid about one foot. Ground guide was bumped with bucket on left hip and buttocks. Ground guide received bruises and was prescribed pain meds. Work restrictions cannot be accommodated by employer.

Struck by falling object

Chain was holding screen panel upright on the floor to the mast of the forklift. Chain had an opened hook securing the chain to itself. The chain was relieved of tension and caused the hook to come loose, causing the screen panel to fall. since then, the chain hooks have been changed to clip hooks for this operation to prevent from coming loose.
