Fall onto or against objects

Tripped and fell backwards, hitting lower back on an I-beam.

Over-exertion in pulling or pushing objects

Attempting to straighten a roll of belting and apparently strained back. Employee did not notify the employer until 06/12/2015 stating that it did not hurt until several days later. and therefore did not seek medical attention until the evening of 06/12/2015.

Over-exertion in lifting objects

Employee was moving and throwing rocks by hand and twisted wrong, straining back.

Trimming a belt from a stacker. Knife came through the belt when finishing the cut and cut right hand through the glove employee was wearing.

Struck by falling object

One employee handing a loader tooth to the other employee and the tooth was dropped on the second employee's left foot.

Contact with heat

Employees were pulling water line on site in hot weather conditions. Employee went into the gen set, sat down and felt dizzy, light headed and complained of both arms hurting.

Contact with heat

Employee was shoveling dirt to smooth a walkway and evidently fainted. The supervisor found him lying on the ground, called his name, the employee woke up but had no recollection of what happened.
