TRAINING IN A TRUCK WHERE THE DRIVER STATES THAT HE LOST CONTROL OF THE EMPTY CAT 773 DUMP TRUCK. THE TRUCK VEERED, RAN THROUGH THE PROTECTIVE BERM AND OFF A 70'-HIGH WALL. (TRUCK LANDED ON ITS TOP; BLUNT FORCE TRAUMA INSIDE CAB OF TRUCK.)

In the course of performing his normal duties, employee thinks he twisted his left hip while ascending and descending the equipment.

Employee was climbing down truck ladder and fell hitting the ground on right side and back.

EMPLOYEE STATES THAT HE LOST CONTROL OF THE EMPTY CAT 773 DUMP TRUCK HE WAS DRIVING. THE TRUCK VEERED, RAN THROUGH THE PROTECTIVE BERM AND OFF A 70'-HIGH WALL. (EMPLOYEE: NEW HIRE IN TRAINING. TRUCK LANDED ON ITS TOP. RESULTED IN PASSENGER FATALITY - BLUNT FORCE TRAUMA INSIDE CAB OF TRUCK; NEITHER WAS THROWN FROM CAB.)

Struck by falling object

EE WAS PREPARING FOR A BLAST WHEN A ROCK FELL FROM THE MINE AND STRUCK HIS SHOULDER AND NECK CAUSING CONTUSIONS. *REPORT BEING FILED LATE DUE TO FURTHER COMPLICATONS ON 10-18-05 EE HAVING FURTHER MEDICAL TREATMENT. AT THAT TIME THE DR PUT HIM ON RESTRICTIONS.

Caught in, under or between a moving and a stationary object

EE was loading tri-axle road tire into bobcat bucket; finger got caught between tire and bucket, causing laceration to right long finger.
