Wrecking forms away from concrete, crow bar slipped and just had a light sprain to left hand wrist.

Employee was changing a tire on HT-222. Tire slipped and left index finger got hung between wheel and the hub. Causing a laceration of finger at the knuckle.

Fall from machine

Employee was inspecting boom on a track hoe. While climbing down the ladder he lost his footing and fell 3-4 feet to ground. He landed on his back and left elbow. Fractured left elbow, contusions to left knee and lower back, Fractured tail bone.

Struck by rolling or sliding object

Reaching to move one piece of sheet metal roofing material, a second piece slid into worker's safety glasses and cut his nose.
