Accident type, without injuries

EE was operating a 785 rock truck when he became disorientated in the fog. He then drove out of the road, across a drill bench, through a berm and over a highwall landing in a shot. The truck then caught on fire and we were unable to extinguish the fire within 30 minutes. EE exited the truck without injury.

Struck against stationary object

Stepped on rock covered with mud and slipped.

Employee was terminated on November 17, 2012 and filed a hearing loss claim. He was diagnosed on 2/26/14 with work related noise exposure. Company Safety Department was not notified until April 10, 2014 of diagnosis.
