Employee was installing roller on D11 dozer, the roller slipped and caught employee's finger.

Employee was diagnosed with moderate to severe sensorineural hearing loss in both ears on September 12, 2013. Company was not notified of this until February 6, 2014.

Claim of occupational noise exposure. Testing shows a degree of hearing loss.

Employee was terminated on November 17, 2012 and filed a hearing loss claim. He was diagnosed on 2/26/14 with work related noise exposure. Company Safety Department was not notified until April 10, 2014 of diagnosis.

EE was operating a 785 rock truck when he became disorientated in the fog. He then drove out of the road, across a drill bench, through a berm and over a highwall landing in a shot. The truck then caught on fire and we were unable to extinguish the fire within 30 minutes. EE exited the truck without injury.

Stepped on rock covered with mud and slipped.

EE was in the process of checking anitfreeze in a 992G Loader. He began to remove the radiator cap and hot anitfreeze came out, striking EE on the chest, stomach and left hand, resulting in a burn to his left hand and stomach.

777 Cat truck was loaded in pit, ground was soft and when truck started to pull out, front of truck raised up then came down hard and jarred operator, hurting neck and back.

Employee had installed a bearing and it was crooked. Was trying to remove it, and while hitting it, he missed the bearing. He was in the process of trying to get his hand out of the way, when his finger struck a sharp object and cut his finger.

Employee was granted 10% award by the Occupational Pneumonconiosis Board.

EE was repairing a rear spring on a F250 pick up truck when the spring fell & struck his left index finger causing a laceration. He rec'd 8 stitches.

Employee was cutting out a blade liner on a D-11R dozer. The liner broke loose pushing him to the ground. His foot was under the blade and his foot was fouled when he began to move causing a break to his lower left leg (blade was on the ground; the ground was uneven where the work was being done).

Employee was pushing in a shot. When he started to back up a rock came from the spoil and he backed up on it. When he started to tram off, the dozer slipped off causing a jolting action resulting in lower back pain. He continued to work until 10/21/06 at which time he bagan to miss work.

EE was starting to climb down the ladder of the 785B rock truck when he slipped and fell to the ground.

Employee was pushing in shot rock. Dozer D-11R he was operating kicked rock from under track causing machine to drop suddenly & resulting in back injury.

Truck driver backed under end loader with bucket raised and rock fell from bucket and hit in empty bed of truck.

EE'S WERE CUTTING TOP OUT OF A 55 GALLON DRUM & AN EXPLOSION TOOK PLACE. THE DRUM WAS BEING CUT INSIDE LOADER BUCKET.

JOB MECHANIC WAS CUTTING BARREL WITH CUTTING TORCH & IT EXPLODED.

EE'S WERE CUTTING TOP OUT OF A 55 GALLON DRUM. AN EXPLOSION TOOK PLACE. THE DRUM WAS BEING CUT INSIDE OF A LOADER BUCKET.

EE WAS GOING UP A GRADE TO DRILL IN A FORD SERVICE TRUCK. THE MOTOR QUIT LETTING TRUCK ROLL BACK AGAINST BERM AND LAID OVER.

EE WAS WAITING ON GETTING LOADED BY A LOADER OPERATOR. THE LOADER OPERATOR WAS GOING TOWARDS THETRUCK WITH A BIG ROCK WHEN LOADER BUCKET DUMPED ON IT'S OWN AND THE ROCK HIT THE EDGE OF THE TRU CK CAUSING A JOLT.

EE WAS CLIMBING UP THE DRILL LADDER AND FELL BACK.

EE INDICATES WHILE GETTING OFF LOADER HIS FOOT \SLIPPED CAUSING HIS LEG TO BEND BEHIND HIM. EE CONTINUED TO WORK. ONCE SWELLING STARTED HE CONTACTED HIS DR. AND SCHED. SURGERY. EE STARTED MIS SING TIME ON 11/22/02.

EE NOTIFIED THIS OFFICE THAT HE WAS SEEKING MEDICAL TREATMENT DUE TO HIS EXPERIENCE ON 10/30/02.HE EXPERIENCED ABDOMINAL PAIN THAT ROTATED TO HIS BACK. EE STARTED MISSING TIME ON 11/25/02, HE WAS TO RTW ON 12/3/02. EE DID NOT RETURN TO WORK& HIS CLAIM IS STILL UNDER INVESTIGATION TO DETERMINE IF WORK RELATED. EE WAS DIAGNOSED WITH A MILD BULGING DISC & DEG. DISC DISEASE.

CYLINDER ROLLED OVER STRIKING EMPLOYEE ON FOOT.

EE WAS WORKING ON COMPANY TRUCK WHEN HE SLIPPED WHILE HE WAS DISMOUNTING FROM THE TRUCK. HE FELLONTO THE CONCRETE FLOOR.

EMPLOYEE WAS RIDING IN A PICK UP TRUCK WHEN THE TRUCK RAN OVER A BUMP IN THE ROAD THROWING EE INTO TOP OF TRUCK CAUSING HIM TO STRIKE HIS HEAD.

EMPLOYEE WAS INSTALLING A BATTERY TERMINAL AND IT EXPLODED.

EE TRAMMED DRILL TO BENCH & STARTED DRILLING AFER SEVERAL HRS OF DRILLING EE COMPLAINED OF BACK PAIN Y

EE WAS GETTING IN GREASE TRUCK & PULLED SHOULDERRIGHT SIDE

OPERATOR WAS OUTSIDE AND WIND BLEW DUST INTO HIS EYE.

BACKED ONTO ROCK IN DOZER. HARD JAR TO RIGHT SIDE.
