An employee was fatally injured when EE was struck by a third-party truck driver who failed to stop at a posted, lighted, STOP sign and made a sudden turn off the primary travel way. Local law enforcement arrested the third-party truck driver and charged the driver in connection with the accident.

Crew was in process of removing counterweights from a Grove GMK6350 crane. During this process, the crane tipped over onto its side while attempting to set two counterweights onto a trailer. This resulted in the crane cab falling onto the side of the first trailer adjacent to the crane pinning the employee's legs. Employee's injury details are described in section 7.
Employee was hooking up vibrator to rail car and a finger got pinched between the bolt on the vibrator and the railcar while they were tightening the bolt causing a laceration and a fracture to the tip of the right pointer finger.
Employee was standing in pinch point on deck of semi trailer when driver pulled forward without noticing employee still on deck. Injured employee suffered a fractured fibula and laceration to right lower leg.
Employee was sorting and stacking Pump hoses and fittings onto a pallet. Employee lost their balance and fell backwards onto tongue/hitch area of a cargo trailer. Employee hit their back on the tongue/hitch area of the trailer.
EE slipped on stairs and hurt back. Incident did not become reportable until 10/18/21 when EE was given restricted duty by Doctor.
Employee got dust in EE's eyes while cleaning up a pile of spilled material. The employee was prescribed medication for EE's eyes when sent for medical evaluation.
EE was tossing a battery to a higher level. When the battery didn't go high enough they attempted to catch it as it fell. In doing this they sandwiched a finger between the battery and a conduit.
Employee rolled ankle while exiting a skid steer.
EE was performing a maintenance job at the primary part of the plant. The shaft for a conveyor pulley shifted downward and made contact with EE's left thumb.
EE slipped while walking to work station at the beginning of the shift. EE did not initially seek Medical attention. Medical requested 02/18/2021. Positive x-ray for fractured rib on 02/26/2021.
Employee was removing final liner from the mill. The hammer dislodged a fragment of the line, the fragment struck the employee's right thigh causing a laceration.
A customer truck driver, collapsed on a catwalk at the loadout area after exiting the safety rack. The cause of the collapse is unknown but, appears to be from natural causes.
