Employee sprained EE's left knee, stepping up and over an air slide. EE's knee was treated as a first aid until 9/9/2025 when a visit to an orthopedic doctor resulted in physical therapy being prescribed to help strengthen the knee.

Employee was stacking steel onto forks of forklift when the steel slipped in their hands causing them to jam finger between forks and steel they were stacking.
Employee was walking to their work area and slipped and fell on wet steel floor.

Wind blew a small speck of debris under the safety glasses of a mechanic's. The incident was reported late and the speck had to be removed with a medical instrument at a doctors office.
Employee suffered beginning stage of heat illness. Provided first aid on sight. Employee chose to go in for medical evaluation and was given an I.V.

Miner was in process of repairing a Rivet Buster in an enclosed area/shop, while taking apart the tool an internal part was ejected from the tool and hit the miner in the right eye causing damage to the miners orbital bone around the eye socket
While attempting to remove a skid steer bucket attachment, AHB's stem man was struck by the bucket attachment during the removal process. The area that was struck by the heel of the bucket attachment, was the EE's left (inner side) leg above the ankle. Debris build-up may have interfered with removal of bucket attachment.

Employee got dust in EE's eyes while cleaning up a pile of spilled material. The employee was prescribed medication for EE's eyes when sent for medical evaluation.
A customer truck driver, collapsed on a catwalk at the loadout area after exiting the safety rack. The cause of the collapse is unknown but, appears to be from natural causes.
EE was tossing a battery to a higher level. When the battery didn't go high enough they attempted to catch it as it fell. In doing this they sandwiched a finger between the battery and a conduit.
Employee rolled ankle while exiting a skid steer.
EE slipped while walking to work station at the beginning of the shift. EE did not initially seek Medical attention. Medical requested 02/18/2021. Positive x-ray for fractured rib on 02/26/2021.

EE was loosening rubber skirting for a belt. EE was loosening a nut the wrench lost hold of the bolt and EE's hand slipped causing the bottom part of the wrench to make contact with the running belt. Once the wrench contacted the belt, the wrench pinched/pinned EE's right middle finger to the metal brace which holds the rubber skirting in place.
Employee performing general cleanup. Began descent of stairs and slipped. EE grabbed a bracket to regain balance and received a laceration of the middle finger on the right hand. Laceration required 4 sutures.
Worker was pushing a wheelbarrow on a semi muddy surface lost footing and dropped to right knee, knee made contact with a stone approximately the size of a lemon, which caused a laceration on the skin below the knee cap.
EE water blasting on tower and has alleged injury to right shoulder while positioning water blasting lance. Additional report of continued pain on 6/17/20. Taken for med. eval. and referred to specialist. Became MSHA reportable on 7/10/2020 with the Specialist positive diagnosis of rotator cuff tear on 07/10/20 and prescribed physical therapy.

EE was working with another employee to unplug a silo and when EE opened door to inspect if material was flowing, cement blew back in EE's eyes. EE was wearing safety glasses and PPE. Debris in eyes, resulted in cornea ulcer on EE's left eye.
Employee was carrying a bucket of grease and felt knee pop. EE was diagnosed with a knee sprain.
Employee was sitting at desk and felt a sting on hand, looked down and saw a spider on the hand. Employee went to doctor the next day and was treated for an infection.

Employee was engaged in operating a portable hydraulic jack. When they and their partner found that they did not have the right size jack, they placed a large nut between the unit and the equipment housing to act as a spacer. When they powered the unit, the nut shot out striking the employee in the right index finger
Employees used a come-along to raise the belt off of the idle roller to take the pressure off and allow for easy change of the roller. Upon raising the belt, the employee found self under the load. The come-along slipped off the frame of the structure allowing the belt to forcibly drop down and strike the employee on the top of the head.
Miner was closing the slag slide on the tower. After 3-4 strikes, the miner's wrist began hurting. Gave employee some ice and refrained from using for the rest of the shift. That morning, EE went to the doctor and was treated as a first aid, wrist sprain. On 4/2/18 EE received a repeat Rx, which created the reportable injury status.
Employee bent down to lift a metal plate, felt a pop and pain in abdomen. Medical diagnosis showed to be a hernia
Employee was guiding a piece of steel plate from a cooler while using a tag line. Employee over-exerted when EE pulled on the line which caused EE to strain a muscle in the chest area on the right side.

EE was working in the clinker cooler during a scheduled overhaul period, specifically, working inside one of the coolers under grate compartments. This is overhead work. The cooler had been cleaned above grate, dust in the compartment fell off a beam and landed in EE's face. EE had the appropriate PPE (protective glasses) fine dust got into right eye and had to be washed out.
Employee cut loose a root brace and put employee's right hand in direct path of pinch point.
IP stepped off of water truck after it was parked. Using three points of control IP stepped off of placing left foot on the ground. IP then rolled ankle causing discomfort.
IP attempted to move a valve across platform, once IP stood up IP felt discomfort in lower back.
In stowing the jib there is a safety pin that was not installed before removing the front pins at the boom.

Employee was attempting to remove a chunk of material caught between a chute and a conveyor with a pry bar. The bar slipped off the chunk contacting the belt this caused the bar to bounce back striking the employee behind his left ear causing a contusion and stiff neck.
Employee was attempting to open a valve for a back up slurry pump, but was unable to get it to open and restarted the primary pump. When pump started an over head slurry line ruptured and sprayed slurry on him blocking his escape route. The employee climbed over the hand rail and jumped about eight feet to the ground causing a fracture/ dislocated right shoulder.
Employee was attempting to free a stuck gate by pulling on it. When the gate broke loose, he got his hand caught by the air cylinder.

Employee was shoveling next to a tin wall in the Clinker Barn. He was not wearing gloves and there was a loose peice of tin still attached to a wall beam. As he was shoveling he brushed his hand against the loose piece of tin and cut his right hand. Employee had to get stitches for his laceration.
Employee was cleaning under #1 Raw Mill and kept hitting his shovel on a metal plate in the floor. Felt fine when he went home, but the next morning his right wrist was swollen.
Employee was working in the Clinker Barn measuring clinker piles when he passed out. Employee was treated for dehydration.
Employee was installing new brackets on stop motion on 432-CV1. The employee bent over to pick up a torch and when he stood up he hit his face on a beam and broke his front tooth.
Employee was moving a 55 gal. drum from underneath a pallet rack. He pulled it very hard and the lid came off. When the lid came off the employee fell backwards into the pallet rack leg hurting his back. When he hit his back it caused a soft tissue contusion to the right flank.
Employee was removing a return roller. He jerked the roller out with his left hand causing the roller to drop, this forced his right hand under the roller and conecting point. He suffered a fracture to his right thumb.

Employee was trying to move the deflector plate @ the headpulley of 351-BC1, it was using a prybar. He stuck the bar between the hopper and the handle to move the gate and the bar slipped, hitting the employee in the head above the left eye.
EE was in the process of replacing a heat shield #4 Kiln, #3 Pier South East trunnion. While in the process of this, the kiln caught the heat shield and trapped his foot crushing it as the kiln came around.
Employee was in the process of repairing an electrical switch in 703 tunnel. He slipped on loose material at the stairs towards the bottom of the tunnel. There were some clinker balls around the stair area as well as dust. He cut his right elbow and received stitches.

Employee called Southern Refractories, Inc. office on August 13, 2004 and informed me that he was having tingling and burning sensations in his knee that was traveling down his leg to his toes. He is at Care Now Medical Clinic today having his leg x-rayed.
