Employee sprained EE's left knee, stepping up and over an air slide. EE's knee was treated as a first aid until 9/9/2025 when a visit to an orthopedic doctor resulted in physical therapy being prescribed to help strengthen the knee.

Employee was shoveling and felt a strain in their shoulder that was treated as first-aid but employee was later given restrictions by a doctor on October 23rd.

Employee got cement dust in their eye causing small abrasion.

Employee was shoveling debris off a catwalk and felt pain in lower back.

Employee was aligning a beam to a column using a bull pin and small sledgehammer. Went to strike the bull pin and missed causing EE's hand to get caught between the column and sledgehammer handle. Sustained a fracture and laceration to the right index finger.

Employee was stacking steel onto forks of forklift when the steel slipped in their hands causing them to jam finger between forks and steel they were stacking.

Employee was walking to their work area and slipped and fell on wet steel floor.

Employee fainted as a result of exposure to high heat and humidity. Employee received medical treatment and was released back to work with no restrictions.

miners were putting new liner in the miner communicated EE was going to release the bar so the Liner would fall into place, Miner had EE's left hand on top of the Liner and did not remove it when Liner was released resulting in EE's middle finger and ring finger on left hand receiving cuts on the tips of the fingers

Wind blew a small speck of debris under the safety glasses of a mechanic's. The incident was reported late and the speck had to be removed with a medical instrument at a doctors office.

Employee suffered beginning stage of heat illness. Provided first aid on sight. Employee chose to go in for medical evaluation and was given an I.V.

Miner was in process of repairing a Rivet Buster in an enclosed area/shop, while taking apart the tool an internal part was ejected from the tool and hit the miner in the right eye causing damage to the miners orbital bone around the eye socket

While attempting to remove a skid steer bucket attachment, AHB's stem man was struck by the bucket attachment during the removal process. The area that was struck by the heel of the bucket attachment, was the EE's left (inner side) leg above the ankle. Debris build-up may have interfered with removal of bucket attachment.

EE slipped on stairs and hurt back. Incident did not become reportable until 10/18/21 when EE was given restricted duty by Doctor.

Employee got dust in EE's eyes while cleaning up a pile of spilled material. The employee was prescribed medication for EE's eyes when sent for medical evaluation.

A customer truck driver, collapsed on a catwalk at the loadout area after exiting the safety rack. The cause of the collapse is unknown but, appears to be from natural causes.

EE was tossing a battery to a higher level. When the battery didn't go high enough they attempted to catch it as it fell. In doing this they sandwiched a finger between the battery and a conduit.

Employee rolled ankle while exiting a skid steer.

EE slipped while walking to work station at the beginning of the shift. EE did not initially seek Medical attention. Medical requested 02/18/2021. Positive x-ray for fractured rib on 02/26/2021.

EE was loosening rubber skirting for a belt. EE was loosening a nut the wrench lost hold of the bolt and EE's hand slipped causing the bottom part of the wrench to make contact with the running belt. Once the wrench contacted the belt, the wrench pinched/pinned EE's right middle finger to the metal brace which holds the rubber skirting in place.

Employee performing general cleanup. Began descent of stairs and slipped. EE grabbed a bracket to regain balance and received a laceration of the middle finger on the right hand. Laceration required 4 sutures.

Worker was pushing a wheelbarrow on a semi muddy surface lost footing and dropped to right knee, knee made contact with a stone approximately the size of a lemon, which caused a laceration on the skin below the knee cap.

EE water blasting on tower and has alleged injury to right shoulder while positioning water blasting lance. Additional report of continued pain on 6/17/20. Taken for med. eval. and referred to specialist. Became MSHA reportable on 7/10/2020 with the Specialist positive diagnosis of rotator cuff tear on 07/10/20 and prescribed physical therapy.

Stepped on a nail.

EE was working with another employee to unplug a silo and when EE opened door to inspect if material was flowing, cement blew back in EE's eyes. EE was wearing safety glasses and PPE. Debris in eyes, resulted in cornea ulcer on EE's left eye.

Employee was carrying a bucket of grease and felt knee pop. EE was diagnosed with a knee sprain.

Employee was sitting at desk and felt a sting on hand, looked down and saw a spider on the hand. Employee went to doctor the next day and was treated for an infection.

Employee was engaged in operating a portable hydraulic jack. When they and their partner found that they did not have the right size jack, they placed a large nut between the unit and the equipment housing to act as a spacer. When they powered the unit, the nut shot out striking the employee in the right index finger

Employees used a come-along to raise the belt off of the idle roller to take the pressure off and allow for easy change of the roller. Upon raising the belt, the employee found self under the load. The come-along slipped off the frame of the structure allowing the belt to forcibly drop down and strike the employee on the top of the head.

Miner was closing the slag slide on the tower. After 3-4 strikes, the miner's wrist began hurting. Gave employee some ice and refrained from using for the rest of the shift. That morning, EE went to the doctor and was treated as a first aid, wrist sprain. On 4/2/18 EE received a repeat Rx, which created the reportable injury status.

Employee bent down to lift a metal plate, felt a pop and pain in abdomen. Medical diagnosis showed to be a hernia

Employee was guiding a piece of steel plate from a cooler while using a tag line. Employee over-exerted when EE pulled on the line which caused EE to strain a muscle in the chest area on the right side.

EE was working in the clinker cooler during a scheduled overhaul period, specifically, working inside one of the coolers under grate compartments. This is overhead work. The cooler had been cleaned above grate, dust in the compartment fell off a beam and landed in EE's face. EE had the appropriate PPE (protective glasses) fine dust got into right eye and had to be washed out.

Employee cut loose a root brace and put employee's right hand in direct path of pinch point.

IP stepped off of water truck after it was parked. Using three points of control IP stepped off of placing left foot on the ground. IP then rolled ankle causing discomfort.

IP attempted to move a valve across platform, once IP stood up IP felt discomfort in lower back.

In stowing the jib there is a safety pin that was not installed before removing the front pins at the boom.

Mill liner came loose and fell three feet and bounced off a liner on floor of the mill, then fell out onto right foot.

EE was hammering slide blank into position, missed with head of hammer pinching left index finger between hammer handle and slide blank. Employee received 7 stitches, fractured finger tip.

EE was cleaning #1 Finish Mill pit and bucket handle broke, bucket fell hitting a chunk in the bottom causing it to ricochet up hitting EE in the left side of EE's face causing a bruise and a small abrasion

Employee was cutting hose with a knife when knife slipped cutting arm

Metal plate fell on employee's right foot just behind the steel toe.

Employee was taking measurements to repair a broken latch on a door, when a cannon blaster in the area discharged blowing hot material in his face.

Employee was attempting to pry a screw loose with a pry bar. When the screw broke loose he smashed his finger between the pry bar and the screw housing.

Electrician was removing an old light fixture an he reached over the top of the fixture his right middle finger came in contact with an old wire tie causing a cut.

Employee was climbing down a ladder, as he stepped onto the ladder he grabbed the safety gate to close it. He had grabbed the hinged side of the gate with his right hand. When he stepped out his weight pulled the gate closed pinching the tip of his right ring finger.

Employee removed his goggles to clean them when the wind blew Alkali dust into his left eye

Employee was attempting to remove a chunk of material caught between a chute and a conveyor with a pry bar. The bar slipped off the chunk contacting the belt this caused the bar to bounce back striking the employee behind his left ear causing a contusion and stiff neck.

Employee was attempting to open a valve for a back up slurry pump, but was unable to get it to open and restarted the primary pump. When pump started an over head slurry line ruptured and sprayed slurry on him blocking his escape route. The employee climbed over the hand rail and jumped about eight feet to the ground causing a fracture/ dislocated right shoulder.

Employee was attempting to free a stuck gate by pulling on it. When the gate broke loose, he got his hand caught by the air cylinder.

Employee was walking up a set of stairs when he felt a sharp pain in his left groin area.

Employee was using a hand drill to tighten wire holding a scaffold plank in place. The tip of his glove got caught in the wire and pulled finger into drill bit. Drill bit cut the 2nd finger of his right hand.

Employee was moving a cooling fan when base of fan came loose, striking his hand.

Employee was getting samples on the pre-heat tower when hot dust blew up his coat sleeve causing burns to his forearms and left hand.

Employee was attempting to free a bag of cement from a conveyor without proper lock out and got his arm caught between belt and roller.

Employee was cleaning spill next to a conveyor belt. He stated he got too close getting his hand caught between the troughing idler and the belt.

Employee was cleaning chunks off of grizzly bars in the clinker cooler with a jack hammer bit when he felt a sharp pain in his right elbow.

Employee was walking down a set of stairs when he stepped on a rock causing a fracture to his left ankle.

Filling bags with portland cement when bag busted open causing cement dust to get into employee's eyes

Employee was carrying a fan shaft up stairs when he felt a burning sensation in his back.

The employee was cleaning the tower and opened a port for inspection. Hot material came out of the port and hit the employees protective hood. Particle dust got under the hood burning the employee on his right cheek, ear, and neck.

The employee was digging out a chute at the CV1 head pulley with a bar. He hit his right hand on a bolt sticking out of the chute causing a contusion to the right hand.

The employee was cutting metal rails out of the 700 tunnel. A piece of metal fell on his left foot causing a contusion.

Employee was unhitching a trailer from a truck. He thought the trailer stand was down. It was not. He tried to keep the hitch from hitting the ground and smashed his finger between the hitch and the ground causing a laceration to the left index finger.

The employee was moving a catwalk. He ran into an I-beam smashing his left pinky finger resulting in a broken bone.

The employee was removing a blade from the separator. The blade fell striking the right forearm causing a laceration. The blade was worn and had sharp edges.

Employee was filling can with fuel and spilt some fuel on pants. The employee then got too close to a smaller container that was lit. His left pants leg caught on fire causing burns to left leg.

Employee was walking down a catwalk, he twisted his ankle and felt a pull on the backside of his right knee resulting in a sprain knee.

After cleaning an air lance with a 1/8th inch rod that was in a vice, the employee went to loosen the vice and slipped on a rock. He tried to catch himself on the vice. He missed and hit his left hand on the rod causing a puncture wound and fracture.

The employee was helping another employee move a plate. One side of the plate fell and caught his finger between the initial plate and another plate sustaining a fracture to the left index finger.

Employee was walking from the main office building toward the parking lot. The employee tripped on the uneven flagstone and fell on the left arm sustaining a fracture to the left humerus.

The employee was leaving the 780 Conveyor headed toward the Storeroom, while riding in the Gator he felt something irritating his left eye. Employee's diagnosis: foreign body removed from his left eye.

Employee was leaning over the catwalk welding a fan frame at #2 Pier on #5 Kiln and strained his back.

The employee was unloading coal from rail cars and some coal got in his eyes. Later that evening he sought medical treatment because his left eye was irritated.

Employee was mixing lime with water for a heat test. He went to dump the sample and the mixture blew out of the container knocking his safety glasses off and getting into both eyes causing chemical conjunctivitis.

Employee was walking and stepped on a rock twisting his left knee. This caused a knee sprain.

Employee was moving a heavy grinding head with a contractor. They stood it up on its side and the contractor spun it around. The employee still had his hand on it and when the head turned it pinched his finger between the head and the frame of a forklift causing the tip of his left pinky finger to be amputated.

Employee was working when his arms started itching and he started to have trouble breathing. Doctors diagnosis: Chemical dermatitis and industrial asthma.

Employee noticed on the completion of her shift that the skin on her chest was irritated. She had been cleaning in the Millroom during her shift. Physicians diagnosis was Chemical Dermatitis.

Large piece of angle iron fell from 35' above.

Lack of altitude fatal plane crash on mine site.

Employee was helping to remove a roller cover. He stepped in front of the cover to see why it would not move when the cover broke free and pinned him between it and the hand rail. The employee suffered contusions to both shins.

Employee was walking down stairs in the Clinker Barn. When he reached the floor he stepped on a piece of Clinker and sprained his ankle.

Employee was inside Tank 10 cleaning when he slipped and fell in debris. While being pulled out of the tank he lost his footing and fell again. Employee sustained contusions to the chest and left elbow and a laceration to the right thumb requiring sutures.

Employee was working in the bagging area and felt a shortness of breath and a skin rash. Employee suffered asthma and contact dermatitis.

The employee was aligning two bolt holes on a screw shaft. He stuck his left middle finger in the bolt holes and asked another employee to turn the shaft. This caused a laceration of the left middle finger. The injury required sutures.

Employee was cleaning debris on top of the clinker silos. A gust of wind blew debris in the employee's eye resulting in conjunctivitis of the left eye.

While installing a chain, the employee was pulling a chain, while other employees were pushing on the chain. The chain released, catching the employees leg between the chain and the chain frame.

Employee was resetting a guard when the guard slipped and caught his finger between the guard and the frame. Employee required sutures.

Employee was repairing the load out chute from a manlift. He was lowering the chute to the manlift basket level, supporting it with both of his arms. He asked the manlift operator to move the manlift forward. The operator moved too far forward and caught him between the chute and the handrail of the manlift basket, straining his back.

Employee was sitting in a Bobcat with his foot hanging out the front. He had the bucket up because he was catching dust. He hit the joystick and the bucket came down on his foot causing a laceration to the bottom of his foot.

The employee was pushing in the wall of a rock feeder with a porta-power and a piece of pipe. The pipe slipped and hit the employee on the forehead. The injury required sutures.

Employee was shoveling and felt a pain in his back resulting in a lumbar strain.

Employee was working on #2 cement cooler. He was turning the slide on the cooler and it slipped and cut his finger which required sutures.

Employee was removing motor from dust collector fan. The motor got hung up on a brace so the employee pulled it harder and strained his back.

Employee was sweeping when he felt his lower back stiffen up. Employee stopped sweeping and sneezed. When he sneezed his back spasmed and he felt severe pain in his lower back.

Employee noticed his rash was getting worse and he needed to see a doctor. Employee has contact dermatitis.

Employee was walking down the steps of the masonry bagging machine. He tripped and fell down the steps resulting in a lumbar strain.

Employee was loading scaffold pipe on trailer.

Employee was shoveling and lifted a shovel with a full load. He then felt a pain in his back and was diagnosed with a lumbar sprain.

Employee was working on a fan. The fan was locked out, but it rolled. When it rolled it caught the employees glove and cut his right hand. Employee's injury required sutures.

The employee was removing a seal on the water spray at #6 Finish Mill. He was prying the housing with a pry bar when the bar slipped and struck him in the forehead. He received sutures in his forhead above his left brow.

Contractor employee was cutting metal on pre-heat tower when metal fell cutting his right hand.

Employee was shoveling underneath a hopper. He felt a slight pain in his groin area. Approximately 45 minutes later he noticed swelling in that area. Employee was diagnosed with a hernia.

The employee was changing the bucket on the bobcat. The bucket was 6 - 8 inches off the ground. He was standing between the bobcat and bucket when he pulled the release pin. The bucket fell on the employees right foot breaking a bone in the right foot.

Employee was shoveling next to a tin wall in the Clinker Barn. He was not wearing gloves and there was a loose peice of tin still attached to a wall beam. As he was shoveling he brushed his hand against the loose piece of tin and cut his right hand. Employee had to get stitches for his laceration.

Employee was cleaning under #1 Raw Mill and kept hitting his shovel on a metal plate in the floor. Felt fine when he went home, but the next morning his right wrist was swollen.

Employee was working in the Clinker Barn measuring clinker piles when he passed out. Employee was treated for dehydration.

Employee was installing new brackets on stop motion on 432-CV1. The employee bent over to pick up a torch and when he stood up he hit his face on a beam and broke his front tooth.

Employee was moving a 55 gal. drum from underneath a pallet rack. He pulled it very hard and the lid came off. When the lid came off the employee fell backwards into the pallet rack leg hurting his back. When he hit his back it caused a soft tissue contusion to the right flank.

Employee was removing a return roller. He jerked the roller out with his left hand causing the roller to drop, this forced his right hand under the roller and conecting point. He suffered a fracture to his right thumb.

Employee was moving metal on to a pallet. His finger was caught between two pieces of metal and he suffered a laceration to the left middle finger.

Employee had a heart attack.

Employee was trying to close the TA Damper when another employee tried to jog it the manual crank caught his right arm causing a laceration to the forearm.

The employee was operating a jackhammer and the air hose got tangled in the 703 pan while it was moving. The employee tried to pull it out and the fitting on the hose cut his finger on his left hand.

The employee was pulling barrels out of pit #2. He felt a pain in his back.

The employee was cutting a gasket at 711-VA1 valve and the razor knife that he was using to cut the gasket slipped cutting the palm of his left hand.

The employee was walking in the 712 tunnel, down an incline. He slipped and fell and twisted his ankle.

The employee pulled the press bed from under the hydraulic press and felt severe pain in his back.

Employee was stepping up the ladder to get into the loader and he twisted his right knee, which caused him severe pain.

Employee was replacing a cement valve over his head. He lined up the valve and pulled too hard and strained his left bicep.

The employee was cleaning under a pan conveyor. He reached inside the guard and layed his hand on top of the pan rail. The pan was running and a steel roller ran over his finger.

2 Employees were driving in a gator on the quarry road and saw a tool in the road. They stopped their vehicle to pick up the tool and were hit in the rear by a pick up truck driven by another employee.

While working on a a feeder chain the employee was holding a pin in the chain and another employee swung a sledge hammer intending to hit the pin and instead he hit the employees hand.

Employee was taking nuts off of tipping valve and felt pain in his neck.

The employee was working on the bagging machine in the Pack House and a bag fell off to the side and pushed his right knee to the far right causing severe pain.

The employee was removing forms inside the riser at #5 Kiln, while doing so he hit a bar between two boards and the hammer bounced off of the bar and hit his hand causing severe pain and swelling.

The employee stepped down into a drainage ditch (between #3 and #4 smokestacks), slipped and twisted his right ankle.

Stepping out of crane, employee heard his knee pop. Caused severe pain and difficulty in walking.

Employee was working on an air compressor on the service truck. There is a step on the side of the bed inside the truck. Employee stepped off of that step and he turned his ankle and it popped.

LOWERING TOOL BY ROPE, KNOT CAME LOOSE, TOOL FELL LANDING ON HAND

Employee was trying to move the deflector plate @ the headpulley of 351-BC1, it was using a prybar. He stuck the bar between the hopper and the handle to move the gate and the bar slipped, hitting the employee in the head above the left eye.

EE was in the process of replacing a heat shield #4 Kiln, #3 Pier South East trunnion. While in the process of this, the kiln caught the heat shield and trapped his foot crushing it as the kiln came around.

Employee was in the process of repairing an electrical switch in 703 tunnel. He slipped on loose material at the stairs towards the bottom of the tunnel. There were some clinker balls around the stair area as well as dust. He cut his right elbow and received stitches.

Employee was driving the loader back from fuel island and hit a large hole. This jammed the seat into his lower back.

The employee was entering truck on driver's side. The weld broke loose on the bottom step. Employee swung down and caught himself with his left knee on the top step. All his weight slammed down on his knee.

THE EMPLOYEE WAS WORKING ALONE IN THE QUARRY PICKING UP WATER LINES, HE SAID HIS LEFT CALF MUSCLE STARTED HURTING BUT HE DID NOT NOTIFY ANYONE ON IT THAT EVENING. THE NEXT MORNING, HE COULD HARDLY WALK SO HE WAS SENT TO COMPANY DOCTOR.

He was cleaning in 700 tunnel when he went to bend down and squat his knee bent to the side causing him pain in his right knee.

Employee cut his finger on right hand while helping move clean up screw behind #2 RM.

Injured was eating lunch with remainder of crew in shade of railcar, when locomotive hooked to cars & started moving. Injured was knocked down and in falling his hand was run over by wheel of railcar.

WHILE GUNITING, EMPLOYEE SLIPPED AND FELL ON ANCHORS AND PUNCTURED HIS BUTTOCKS.

Employee grabbed a shovel and got his thumb caught between the shovel's handle and a locker. When employee pulled his hand away, he cut his thumb and pulled his thumbnail off.

Rock from tower fell and struck employee's left knee. Employee called Southern Refractories, Inc. office on August 13, 2004 and informed me that he was having tingling and burning sensations in his knee that was traveling down his leg to his toes. He is at Care Now Medical Clinic today having his leg x-rayed.

EE ALLEGES THAT WHEN HE OPENED 432 DUST BIN A LOT OF DUST CAME OUT AND HE COULDN'T SEE. HE TRIPPED ON SOMETHING AND FELL ON CATWALK, CUTTING HIS RIGHT KNEE.

EMPLOYEE ALLEGES THAT WHEN HE AND TWO CO-WORKERS WERE LIFTING A SNUB ROLLER, HIS FINGER GOT STUCK IN BETWEEN THE SNUB ROLLER & THE CATWALK PULLING OFF THE TIP OF HIS LITTLE FINGER.

EMPLOYEE ALLEGES THAT WHILE WORKING HIS PANTS GOT WET FROM RAIN AND RUBBED SORES ON HIS LEGS. **NOTE**EMPLOYEE DID NOT NOTIFY HIS SUPERVISOR UNTIL 1/23/04.

EMPLOYEE ALLEGES THAT WHILE HE WAS ROLLING UP AN AIR HOSE, THE HOSE CAUGHT ON A PIECE OF STEEL CAUSING THE STEEL TO FALL ON HIS FOOT.

EE WAS LOWER LOUVER BLADES THE NIGHT OF THE INJURY HE S NOT QUIT SURE WHEN BACK SPRAIN HAPPEN. HE WORKE UP ON THE 28TH WITH A PAIN IN HIS LOWER BACK.

EE ALLEGES THAT WHILE WORKING ON #2 SEPERATOR, HE BECAME OVERHEATED.

EE ALLEGES THAT WHILE HE WAS LINING A 5-15/16"" SHAFT IN A PULLEY, HE FELT A SHARP PAIN IN HIS BACK.

EE ALLEGES THAT WHILE HE WAS HOLDING A BACK-UP WRENCH FOR A CO-WORKER WHO WAS TIGHTENING A BOLT HE FELT A SHARP PAIN IN HIS NECK.

EE ALLEGES THAT WHILE CHANGING A VALVE, HE FELT A BURNING SENSATION ON HIS LEFT SIDE. NOTE: EE DID NOT SEEK MEDICAL ATTENTION UNTIL 6/14/03.

EE ALLEGES THAT WHILE USING A COME-A-LONG ON A MOTOR, THE COME -A-LONG FELL HITTING HIM IN THEMOUTH.

EE ALLEGES THAT WHILE HE WAS AIR LANCING, CEMENT DUST SHOT IN THE SIDE OF HIS SAFETY GLASSES. NOTE: EE DID NOT TURN IN UNTIL 5/26/03 AND DID NOT SEEK MEDICAL ATTENTION UNTIL 5/27/03.

EE ALLEGES THAT WHILE WORKING IN #703 TUNNEL, HE GOT CEMENT DUST IN BOTH OF HIS EYES.

EE ALLEGES THAT WHILE LIFTING A GUARD, HE FELT A SHARP PAIN IN HIS BACK.

EMPLOYEE WAS REMOVING GRATES THROUGH BOTTOM ACCESS IN FINISH MILL AND A PIECE OF LOOSE STEEL FELL THROUGH THE ACCESS AND STRUCK EMPLOYEE ON RIGHT FOREARM. INJURY ATTENDED PHYSICIAN ON 1-26-02 BECAUSE OF INCREASE IN PAIN.

EE WAS ENTERING A VERTICLE ROLLER MILL FOR INSPECTION. EE STEPPED INTO THE OPEN GAP BETWEEN THE ENTRY AND THE ROLLER TABLE, CAUSING HIM TO FALL UP TO THE THIGH ON HIS LEFT LEG AND STOPPIN GHIS FALL WITH HIS BARE HANDS ON THE HOT SURFACE TABLE. EE SUFFERED AN ABRASION TO HIS LEFT SHIN AND 1ST/ 2ND DEGREE BURNS ON HIS HANDS. EE WAS NOT LOOKING WHERE HE WAS STEPPING, GLOVES NOT WORN.

EE ALLEGES WHILE DRIVING HE HIT DUCT WORK AND TIPPED THE TRUCK OVER CAUSING CONTUSION TO HIS THIGH.
