Accident type, without injuries

A rock outburst in the 3700/244 stope caused the withdrawl of miners from the work area following one miner be hit by rock from the outburst while rock bolting at the face.

Rockburst occured in crosscut during the day shift, caused withdraw of the miners in work area.Normal activity was disrupted for more than one hour and MSHA was notified. Rockbust created about 6 cars of muck at the face, a couple of smaller burts followed and consensus was to back away for the remainder of the day.

Employee was attempting to unhook a track mucker from the train when the employee got his foot between the wheels and was run over by the mucker.

Employee was helping lower concrete forms for the next shaft pour. The form became hung up and then sprung free crushing the employees hand between the steel form and the concrete shaft.

The employee was rockbolting the back when a rock fell catching the employees left hand between the jackleg and the falling rock, lacerating and fracturing his index finger.

Rerailing a 12B track mucker and pinched finger requiring sutures.

Employee was standing in the mud of a sump pulling on a pump hose. It broke free and employee could not move his feet causing him to wrench his back.

Employee was moving supplies at face when he stepped on an oily air hose causing him to slip and twist, straining his lower back.

Employee got his fingers in a pinch point trying to rerail a 12B track mucker.

Employee stumbled on a rock and hit shin on slusher bucket causing a large hematoma.

Employee doing track repair, bent down and couldn't get up. As of January 27, 2009 this case was denied by the State of Idaho that the employees condition was not the result of an industrial injury but a continuation of a pre-existing condition.

Employee was unloading rockbolts when the metal band on the bundle broke striking the employee in the Rt. Forearm causing a deep laceration.

Employee was standing on the third rung of a ladder. Employee was attempting to lift and move a pan feeder segment. When he stepped down his foot went between the second and third rung causing him to fall straining left knee.

Employee lifted wrong straining his back while changing steel during drilling.

Employee was knocked down from a rock burst. Employee said he was ok and returned to work the next shift. On 7/11/08 employee asked to see a doctor for his rt. elbow and was released full duty. On 7/30/08 employee revisited the doctor for his elbow and was placed on restricted duty and physical therapy treatments.

A rock outburst in the 4600 level Decline ramp caused the withdrawl of 2(two) miners from the work area during the rock bolting cycle of the mining cycle. One (1) miner sustained minor injuries.

Miner was drilling with a stoper when a small rock fell causing a laceration on right wrist requiring sutures.

Employee was drilling at the face. Steel broke at the chuck tender causing machine to lunge forward. Employee lost his balance and struck face with left shoulder.

EE was ascending raise ladder to stope area. bear the top the EE encounted a damaged ladder rung. The rung pulled loose causing the EE to fall to the muck pile below.

Employee got debris in left eye while mending a slusher cable.

While cleaning bulkhead, rock fell from rib and struck employee in left wrist.

While drilling, the leg malfunctioned. The leg knocked off the employee's hard hat, and the drill steel struck him on the top of his head.

Employee was bolting the back in the stope when a rock fell striking the employee on the left shoulder causing a laceration requiring 3 sutures.

The 46-148 crew were moving a charge drill hose when the valve on the oiler was pulled open causing the hose to whip, striking the employee in the right side of the head.

EE was using knife to open a box, EE slipped using the knife causing a laceration to his left hand. EE needed sutures.

EE was loading bolting mats into cage. The EE lost control of the mats he was loading, catching his hand between the mats and cage door. Injury was a cut on index finger, requiring 2 stitches.
