Accident type, without injuries

No. 3 Hoist, rope stretched on #2 side, brake would not release. Reset computer, brake released, period of time to find out why brake would not release. Hoist back up on line at 1130 hrs.

Low battery alarm at 10:20 AM supervisors stood miners down - electricians replaced 4 AA Batteries and contacted Canada on computer backup

Galena Hoist - Hoist went down at 2030 hrs. due to blown fuse.

No. 3 Hoist getting ready to send men underground the left brake limit switch was stuck; computer would not release till it was reset.

Galena hoist was parked when Hoistman tried to move the hoist the brake mechanism would not release. East brake control relay was weak, replaced relay and back up and running

Galena hoist parked when there was a power bump. Brake mechanism would not release. After repeated attempts to release brake decided to release brake manually to compress cylinder. This worked and hoist was back up and running.

5200-165 using jackleg, leg kicked out miner tried to catch it and felt burning in lower abdominal area. Went to ER on Saturday, Sept 24th. To specialist in Coeur d'Alene

4900 Chiller - was moving circulating pump, pump tipped over - EE tried to catch it. Strain to mid abdomen. Reported accident on Monday, Sept 26th. To specialist.

Over-exertion in lifting objects

3400-51W - bent over to pull jackleg out of lifter, felt like tear to abdomen. Was seen on Monday, August 8th. Will be seeing a specialist in Post Falls, ID this week.

Caught in, under or between a moving and a stationary object

Knock rock out of drill hole using rock bolt and double jack, hit rock bolt with double jack, smashed left little finger between rock bolt and double jack

Over-exertion in lifting objects

Men's Dry, clean men's dry, lifting buckets and mopping, abdominal strain - hernia surgery scheduled for Friday, July 8th.

Struck against stationary object

Bumped elbow going up raise, saw doc on 3/23/16. Released to full duty. Re-examined on April 5th, infected. Off work till infection clears up

Struck by falling object

Miner pried rock off muck pile to move 15 hp. slusher up raise, rock slid down and hit miner in left shin.

Over-exertion in lifting objects

Use 6-foot bar to move mill pinion shaft (approx. 3,000 lbs.) across shop floor, overexerted - abdominal strain - hernia repair June 1, 2016

Absorption of radiations, caustics, toxic and noxious substances

At 4900-167-Cell #3, during a cement-sand fill, support for wall failed knocking miner backward, resulting in minor cuts and bruises and chemical conjunctivitis.

Stairway from shop to main deck, employee move ladder from walkway to storage, heel of boot caught on bottom step causing ee to trip and twist ankle

Unload slusher off timber truck, hoist slusher off timber truck, slusher swung out - miner had hand between slusher and hoist cable

Galena Mill - #2 belt scraper, tighten scraper, bar used to tighten scraper slipped employee slipped fell into metal ladder rung cutting eyebrow above right eye. 4 sutures

4900-360 Collaring hole to drill for roof bolt, material came out of back hitting bridge of nose.

5200-206 - Remove utilities from 206 intersection. Had timbers on bucket. Exited mucker and went to unload bucket by hand. Slipped on bucket/timber knocked into rib.

Diamond drill in 2400-181; leaned hand against rib to stoop down to grab something; cut left wrist on sharp rock edge on the rib.

Employee greasing hoist coupler in Galena Hoist room, caught left wrist on sharp edge of machine guard.

Struck by flying object

Cutting dag with cut off wheel, got piece of metal in left eye. Emp. was wearing wrap around safety glasses.

Diagnosed with Carpal Tunnel Syndrome, both wrists. Surgery on 1st wrist - Wednesday, June 1st

Employee claimed carpal tunnel syndrome, diagnosed March 1st - surgery on Tuesday, May 24th.

Miner had mild heart attack - transported to Coeur d'Alene, ID - had two stints placed and has been released to full duty next week, November 14th.
