Accident type, without injuries

While operating the Volvo 550 excavator a hydraulic leak started during operation that led to a fire in the engine compartment. The operator observed the fire and safely exited the equipment.

Over-exertion (Not Elsewhere Classified)

The employee was working on mineral separators when they stepped on a mesh cleaning ball and twisted their right knee. They did not fall but, felt soreness the next day.

Struck by rolling or sliding object

The employee's leg was struck by a shifting slurry hose while disconnecting a coupling. They went to a doctor and was diagnosed with a tear in their ACL. The injured employee had surgery on 9/4/25 which made this injury reportable.

Over-exertion (Not Elsewhere Classified)

The employee was walking down the steps from the south Ottawa mine screenhouse when EE slipped and fell. EE caught self with EE's right hand on the handrail and felt immediate discomfort in EE's right shoulder.

Fall to the walkway or working surface

The employee fell while traversing an irregular surface. They put their arms out to catch themselves and sustained a closed fracture to the left radius and ulna.

Fall from machine

While cleaning a back-up camera, employee fell approximately 3 feet from the side ladder, striking EE's left shoulder. The employee received only diagnostic care until January 21, 2026, when EE began physical therapy.

Struck against stationary object

Employee struck the back of EE's left hand on metal supporting structure while ascending stairway. Resulting in a fracture to the third metacarpal of the left hand.

The employee sustained a left forearm laceration while using a pair of scissors as a draw knife to cut scrap cardboard into smaller pieces for disposal. The employee was pulling the scissors toward themselves when the scissors slipped and cut into their forearm, 4"" above the wrist and glove, causing a laceration requiring 4 stitches.

Over-exertion (Not Elsewhere Classified)

EE had partial distal bicep tendon tear in the right arm requiring surgical repair. Injury determined to be the result of tendinitis
