Over-exertion (Not Elsewhere Classified)

EE WAS OPENING LID ON RAILCAR - HURT LOWER BACK INTO LEFT LEG. EE WAS BENT OVER THE LID AND THE LID WAS HALFWAY OPEN. TREATED AS FIRST AID UNTIL 12/13/04.

Ee stepped in a hole between tracks 4 and 5 south end, twisting his right knee.

Struck by flying object

Employee was over valve, starting to open the 10' valve when victalic coupling blew apart due to the mainline being under presser 200 psi the force of the uncoupled pipe hit employee on right shoulder and ribs, knocking him to the ground.

Struck against a moving object

EMPLOYEE WAS PUSHING OVERBURDEN TO EXPOSE SAND PRODUCTION. IN DOING SO, HE FELT AND HAD PAIN IN SPINE AREA. THE DOZER IS ONE OF THE WORST MACHINES RE: JERKING, THUMPING, HARD ON OPERATOR. LOADER CAN BE ROUGH ALSO.

EE WAS STEPPING DOWN FROM A PLATFORM WHEN HE TWISTED HIS LEFT LEG/KNEE. FURTHER DIAGNOSIS REVEALED TORN LIGAMENTS AND EE UNDERWENT SURGERY 7/7/04.

CLIMBING STAIRS TO 2nd STORY IN WAREHOUSE, HIT RIGHT KNEE ON STAIR POLE.

Over-exertion in pulling or pushing objects

THE EMPLOYEE WAS ATTEMPTING TO TAKE A PUMP CASING OFF FRO REPAIRS. THE EMPLOYEE WAS USING A 3/4"" DRIVE RATCHET TO BREAK CASING NUTS LOOSE & WAS PULLING UP ON THE RATCHET WHEN HE FELT PAIN IN HIS LEFT GROIN AREA.

Struck against stationary object

He was closing a valve using a pipe wrench. The wrench slipped causing him to hit his left hand on a support beam and lose his balance twisting his left knee.

Employee was removing heat tape from tall oil pipe. Employee's rib cage was leaning against the handrail. When employee reached over the handrail to pull the heat tape off, pressure was exerted on his right rib cage resulting in fracture.

While working on a broken water line in front of wet process, he had dug a ditch to expose the break. He needed to get to the far side of the ditch. He stepped across. The sand on the far side of the ditch was soft and when stepping across the sand gave way under his weight and he strained his left calf muscle.

Walking to mill, slipped on ceramic balls, bruised left shoulder.

Struck by flying object

HIGH WINDS ON TRACK CAUSING AIRBORNE MATERIAL (SAND). EE'S EYE VERY SCRATCHY. EE SAW DOCTOR AND RECEIVED DROPS.

EE WAS STANDING ON THE DECK OF THE BLUE WORK BOAT, SPOOLING OUT CABLE FROM THE BANK. HE WAS CLOSER TO THE EDGE OF THE BOAT THAT HE REALIZED. AS HE TURNED, WHILE HOLDING THE CABLE, HE SLIPPED OFF THE EDGE OF THE BOAT. AS HE WAS FALLING INTO THE WATER, HE GRABBED THE HAND RAIL WITH HIS RIGHT ARM.

Over-exertion in lifting objects

Lumbar strain occured during normal work activities as a Micron Bagger. This entails lifting and turning with up to 50 lbs. of bagged material.

EE WAS COMING TO WORK AND WENT TO ENTER LAB DOOR. WHEN HE REACHED FOR DOOR HANDLE HE MISSED STEP. HE THEN LANDED ON NEXT STEP WHILE STILL HOLDING ONTO HANDLE. HE THEN FELT PAIN IN HIS LOWER BACK AND HIP AREA. (IMPROPER LANDING AND STEPS CONTRIBUTED TO ACCIDENT.)

Over-exertion in lifting objects

Injured right wrist doing normal activities of processing whole grain sand samples. Was lifting buckets and splitting the samples down to run screen analysis.

EMPLOYEE WAS CUTTING A PLASTIC TIE TO REMOVE A BAG ATTACHED TO A DRAINPIPE COUPLING. POCKET KNIFE SLIPPED & EMPLOYEE CUT HIS LEFT WRIST, REQUIRING 4 STITCHES. NO RESTRICTIONS FOR WORK.

Inhalation of radiations, caustics, toxic and noxious substances
