Caught in, under or between a moving and a stationary object

Employee was assisting repair/replace toggle plate and toggle plate bearings on the jaw crusher when they suffered a pinning type injury.

Struck by falling object

While performing maintenance a bearing moved unexpectedly allowing the pulley to drop resulting in amputation of the tip of the employee's finger.

Employee was in the process of reinstalling a roller frame on a dozer. As the employee was sliding the frame back on the dozer a bogie slid and fell onto the employee's left hand.

IW was in chute above P4 tail pulley when EE's hand was impaled with a piece of steel that was sticking away from the wall causing a laceration to EE's right hand.

EE claims they injured their left shoulder while climbing out of a chute. This incident did not become reportable until 12/5/2023 due to the employee going to the doctor on 12/4/2023.

Struck by falling object

Employee was in the process of cleaning out head boxes when a softball size clump of material fell approx. 3-4 feet and hit the employee on the hard hat.

Employee was exiting a 988K loader when EE's foot got caught on the second step causing EE to fall approximately 2-3 feet to the ground fracturing hip.

Struck against a moving object

A Komatsu 605 haul truck backed into a Cat 775G haul truck. The right rear corner of the bed of the 605 contacted the left side handrail and front left cab support of the 775G. EE in the 775G was transported by ambulance to ER claiming pains in their back.

Employee alleges that while operating the water truck on 8/25/23, employee hit a bump causing a back pain. Employee did not report the incident until 09/05/23 and sought medical treatment on 09/06/23. While this incident is still under investigation, this 7000-1 is being reported within the 10 business days of receiving medical treatment.

IW left pinky finger got caught between the drive belt and sheave resulting in a laceration.

Employee was struck in the mouth by a falling rock while raising a screen panel, breaking a tooth.

While cutting tube with a box knife the driller cut self with the knife and EE left the quarry without telling anyone what happened. Once EE arrived home, EE realized it was more than EE could take care of so drove self to a medical facility receiving 4 stitches to inner calf on EE's right leg. EE was released back to work with no restrictions.

An EE went to get some bolts at the bolt room and IE was tightening the rest of the bolts when EE came back IE was walking around EE's truck and IE called EE over and told EE that IE had just fell down.

While cleaning under the conveyor belt, the employee's arm and shoulder contacted the conveyor frame, causing a skin tear on the left elbow and acute pain in left shoulder.

Employee had a personal medical condition causing loss consciousness and falling to the ground and fracturing orbital bone.

Over-exertion in pulling or pushing objects

EE went to throw the rail switch when EE felt a pop in right arm. Did not seek medical treatment until 4/8/2023

Employee alleges the seat in 29062 EH 1600 was ""bottoming out"" when hitting road bumps. Employee alleges neck and shoulders strain.

EE was using left hand to wipe dirt off excavator fuel filter part number when EE cut left index finger on the fan.

Employee fell exiting a skid steer.

Employee was struck by a chain on EE's lower left leg which resulted in two fractures that required surgical repair.

Employee was standing in the doorway of the equipment room talking to a co-worker and EE turned to walk away and when EE took a step, EE heard a pop and felt a burning pain in EE's right foot.

IW was descending walkway beside the wash screen carrying a small hose when they stepped onto a rock and tripped forward down two steps and landed on their left hand.

""Was in screen on back trying to move a screen, twisted awkwardly and lost control of screen. Felt a pop on knee, had discomfort but no swelling or bruising.""

Employee had arrived at work and pulled into the scale house parking lot and was exiting EE's vehicle to throw away personal trash from home when EE fell exiting EE's vehicle.

Employee was removing a clamp from a DEF hose on a CAT D8T dozer. The clamp broke loose and the employee felt a pop in EE's right shoulder.

Employee was setting the water jacket in a 7 foot crusher. The water jacket shifted and pinned the employee's leg between the water jacket and socket causing a fracture.

The employee was attempting to open the ladder for the 57's catwalk when EE felt a pop and severe pain in EE's left arm.

Employee was reaching into the back of a pick up truck to remove an object, when doing so employee felt a pain in right shoulder.

EE was lifting a roller during maintenance on a conveyor when EE strained EE's left shoulder.

Employee was completing a final drive rebuild for a 745 axle. The final drive had already been disassembled and laid out on table for cleaning. Employee went to stand spindle up on table when the spindle slide and caught employee's left index finger between the spindle and table.

Employee was attempting to shovel material underneath a conveyor when EE felt a pop in their left shoulder.

Fall onto or against objects

Employee was making their way to the back of a locomotive via the walkway and slipped on a step. They landed on their left knee. Later in the morning employee was still feeling discomfort and asked to go to the clinic. Employee was evaluated and referred for further evaluation and given light-duty restrictions.

EE was dumping metal 55 gallon trash can into dumpster. Dumpster height is 32 inches. EE described lifting the trash can up and feeling a pop in right bicep.

Employee was cleaning out around the primary area and stepped on wet material and slipped. Employee landed on their lower back, right side on the last step of stairs.

Employee was walking into the main office, tripped on the front steps/rug and hit knee on the concrete platform.

Employee was performing maintenance of sump pump/slurry tank of Scavenger Cyclone. Employee stepped onto a pipe, lost footing, and slipped, causing them to strike their shin on the pipe. This caused an abrasion and contusion to the left shin.

EE was climbing down from the loader and fell backwards, resulting in a fractured metatarsal of the right foot.

Employee injured hand while dislodging oversized rocks from a chute. EE went to the ER, received 5 sutures and is currently working on restricted duty.

Struck by falling object

Employee was assisting with the installation of a new seven foot cone crusher. As the crusher was being lowered one of the bolts was pushed up about six inches and skewed to one side. The employee grabbed the top of the bolt to realign it with the bolt holes and as it lined up the hole it dropped on the employee's thumb causing laceration.

Employee was riding in the buddy seat of an articulating truck when the truck hit a rock, jarring the employee's back.

Fall from machine

Employee was loading customer trucks with a John Deere 350 excavator. As they were exiting the machine they lost footing, injuring their right ankle.

Employee squatted down to pick up a screw, injuring their left knee. Prescribed physical therapy on 9/14/2023.

EE was changing a turbo on a haul truck. EE was using a scraper to remove the gaskets from the turbo and needed to change the blade. After changing the razor blade on the scraper, the employee was trying to tighten the set screw with an O-ring pick. EE's hand slipped and caused the razor blade to cut EE's middle finger on EE's right hand just above the knuckle on EE's right hand.

Employee was replacing the idlers on the CAT 289D skid steer. As they pulled one bolt out and proceeded to the other bolt the idler fell. The employee was trying to stop the idler with their hand and their right index finger got pinched between the idler and the frame of the machine.

EE was replacing the protective wrap on an electrical cable on the sand dewatering screen. The wrap was secured to the cable with zip ties so EE used a Dewalt razor knife to cut the zip ties to remove the protective wrap. While attempting to cut one of the zip ties with the knife in right hand, the knife slipped and EE cut the back side of left hand. EE received 8 stitches.

Employee slipped in some mud while fueling employee's drill and twisted left knee. Employee sought medical attention on June 27, 2023 and was diagnosed with a sprain and released to full duty. Employee returned to the doctor on July 7, 2023 for a follow up and was prescribed physical therapy, continuing full duty.

On 12MAY23 EE slipped while using a pressure washer. EE grabbed a nearby support beam to prevent falling. EE reported incident on 15MAY23. Requested medical attention on 16MAY23. Diagnosed with right rotator cuff strain.

While drilling out a cable fitting the drill bit pierced the employees glove and the outer portion of their left thumb. The employee received stitches.

Caught in, under or between a moving and a stationary object

While torqueing wheel fasteners on a 785 truck the technician placed hand in the path of the reaction arm causing the hand to be crushed between the arm and the wheel.

Struck by flying object

OTR technician was repairing chains on a Loader. EE was cutting off a chain link and the cutting wheel shattered and a piece of the wheel struck the technician on the shoulder causing a laceration.

Over-exertion in lifting objects

Employee was lifting a 992G loader shank into the loader bucket. The employee went to release the shank and felt a pop in lower back.

Struck by falling object

Employee was cleaning beneath the grizzly, at the dump hopper. When a clumped mass of material fell through the gap in the plate steel shielding the top grizzly, dropped approximately 15 feet, striking the employee on the top rear of their hardhat. Employee sustained a concussion without loss of consciousness and a neck sprain.

Employee had finished washing out and was descending a ladder when EE's right foot slipped off the bottom rung causing EE to drop approximately 16 inches to the platform where EE rolled right ankle when EE landed.

The oxygen and acetylene bottles in the rear of the repair truck shifted. As the employee tried to reposition them, the weight shifted causing injury to EE's right little finger and ring finger. EE received 6 sutures is currently on restricted duty.

Struck by flying object

Employee was changing blow bars on the portable plant, using a soft faced sledgehammer, when a metal fragment came off the shoe causing a laceration on the anterior of EE's right elbow. EE sought medical attention, first aid was rendered and released to full duty. EE had surgery on March 1, 2023 to remove the metal fragment from EE' arm and is currently working with restrictions.

Employee was lifting up EE's right leg as EE was about to step over a loader rut at the facility. As EE lifted EE's leg in the air EE felt pain in EE's right hip. Employee went to ER on 1/12 for pain. Employee visited the clinic and was given restrictions on 1/13.

While doing maintenance on a crusher, employee was struck on the chin by a bolt that fell causing a laceration.

Employee closed the door to the primary switch house on EE's thumb.

Employee claimed that employee twisted an ankle while transitioning from a set of stairs to the ground. While this incident is still under investigation, this 7000-1 is being reported.

EE was using a Metabo with a 6" cutting wheel to cut a piece of 2" flat bar. While cutting the flat bar the Metabo kicked back off of the flat bar and the wheel struck EE in the leg just above EE's knee. EE was taken to the emergency room and received 5 staples. EE was released to return to work and will return to work the next work day.

IW was attempting to gain access to the scale office when EE noticed the door was blocked by a portable trash receptacle. EE then grabbed a chair and attempted to climb over the ½ door to the office. On EE's way over the door the top board on the ½ door broke and EE fell to the ground landing on EE's ankle. Did not seek medical treatment until 9/18/2023

IW was grinding on W13 drip pan on 8/14/23. At some point between 9:00AM and 12:00PM they felt something in their right eye. They washed it out on the night of 8/14/23. Employee did not seek medical treatment until 8/16/2023.

Employee was walking around a maintenance truck not paying attention and walked into a stick of drill steel that was laying on the closed tailgate of the truck chipping their left front tooth.

Employee was under a F-250 pick-up finishing up tightening the tab bolts for the cross-member for the transmission. While in the process, one of the tab bolts spun loose and caught the EEs palm of EE's hand.

While retrieving a Rock Hog 4.5"" DTH Hammer from the pickup truck, EE realized the hammer wasn't operating properly. Co-worker told EE to slam it onto the ground to remove the hammer internal parts. This is when EE's hand went between the bit and the chuck. This movement is what caused the injury to EE's hand.

Cooling press off and fitting blew out. The steam started burning and EE couldn't see and ran away into a pipe.

Employee was operating a crane when it tipped to its side. The operator jumped out of the crane landing face first striking their forehead causing laceration needing stitches.

IW was using a hammer to drive a pin on a set of tracks when a piece of material broke off pin and struck IW's right arm

Employee was cutting a piece of wear metal out of a chute. As employee was cutting, the 42-inch x 21-inch x 1-inch plate popped free and smashed their left thumb between the chute and the plate.

Employee placed the loader bucket pin on top of the bucket. Employee then bent down to guide the operator to properly line up the bucket/boom arm hole. The pin rolled off the bucket and hit the employee's head.

While switching plant over employee needed to use a come along to flip a gate, once the come along was grabbed the chain was knotted up. The employee hung the come along up to straighten the chain and get some slack in it, when EE pulled the slack the hook came free and struck employee in the mouth breaking one front tooth in half.

Employee was loading a broken heel plate from a loader into the back of a service truck. EE cut left pinky finger on the plate as EE tossed it into the bed of the truck. EE received 8 sutures and was released to full duty.

While in the process of hand scaling a highwall, a rock chipped off the wall, striking the employee in the face.

Employee was in the process of using a come-along to hoist a section of angle iron which was to be welded to a conveyor belt head roll structure for bracing. As they tensioned the device and were setting the brace in place, the hook slipped up the brace and pinched their right middle finger between the hook and the brace. This resulted in a laceration to their right middle finger.

Employee's hand slipped off wrench while removing screen bolt.

EE was changing a bulb on the R-85 haul truck. There is a rubber grommet around the lens which needed to be removed. EE pulled out EE's pocket knife to cut the rubber. While EE was putting it away EE's pinky was caught in the pivot point of the serrated portion of the blade. This resulted in a cut on the outside of the left hand pinky from the nail around the finger pad.

Employee became over heated while working outdoors and was taken to the hospital where EE received 1 bag of intravenous fluid and Tylenol for headache.

Employee was cutting and welding in a loader bucket and the employee got too hot and suffered a heat related illness.
