Two contract employees of G & H Mechanical Services welded a 3.5 X 6 inch patch on the outside of the collection chute between the 3A Screen tub and the flume around 7:30 pm. The contractor stated somewhere around 8:10 pm they noticed flames on the 3A wash screen.

Contractor was replacing belt scraper, while using a torch a hot slag fell onto a Poly Screen. The fire department was called to extinguish. The initial report was that the fire was extinguished in 20 minutes. When the fire report was requested and reviewed it was stated on the report that it took 45 minutes to extinguish.
Employee stepped out of the cab of the truck to clean the mirrors when EE noticed the truck was rolling. While re-entering the cab, EE's left hand was pinched between the cab handrail and the truck bed in front of EE.
Employee was installing a new air conditioner and alternator on a loader when EE's finger got caught in the belt, amputating the tip of EE's left middle finger up to the nail bed. Employee sought medical attention and is currently on restricted duty.
EE was installing the hammer of the PC490. The employee was using a bar in a swinging motion to push the pin out of the boss on the stick. The pin was almost out as the employee was swinging the bar toward the pin. The pin fell out and the PC490 stick pushed downward on the bar causing the end of the bar to rapidly move upward striking the employee's head.
Employee was trying to get into the bucket truck when employee's right leg slipped and folded employee's left knee into a squatted position. Employee did not go to the doctor until the following day.
EE was placing a track pad when EE felt a slight strain in their neck.
Employee tripped over a rock on a footpath that leads to the highwall. Employee injured shoulder as EE fell to the ground.
Employee was clearing rocks under the standard crusher when a large rock striking the employee's right pinky and ring finger.
Employee cut EE's finger while changing wear plates on the 988.
As the employee was using a skid steer to move grits in the plant the machine hit a rut and bounced and the employee claims it jarred EE injuring EE's back.
Employee was going to unstop a chute and went to turn the water supply valve on. The hose busted spraying the employee with hot water burning EE's left arm, stomach and left leg.
Employee was in the process of moving a 6' x 4' wire screen. Employee lifted the first screen to move it out of the way to get to the next screen. While lifting the screen and turning employee felt pain in left arm.
Employee was drilling a hole through the frame of a hydraulic press to install a guard when the drill caught and spun causing the employees hand to spin and EE's index finger struck the metal bracket causing the laceration.
Employee was helping install a cone crusher ram cylinder when it slipped and caused a contusion to their left hand.
Operator was climbing down from machine and was using 3 point contact and missed the last step and fell to the ground and fractured their right ankle.
Employee was descending from being on the tracks on CAT Dozer, when employee stepped down on the dozer arm employee's foot slipped forward causing employee's leg to go down between the track frame and dozer arm causing the employee to fall backwards to the ground with employee's right leg stuck causing a fracture.
Employee had finished using a cleaning solvent and lit a cigarette. Ashes from the cigarette caught residual solvent on the employee's shirt on fire.
EE performed work in chute, climbed to top of screen deck, slipped and fell (slid) head first 25' through the screen and down the chute.
Employee was working in a chute when they lost footing and fell approximately 2-3 feet. They landed on top of the screen deck hurting their foot.
Driller was out of EE's drill checking holes. The ground conditions were very dry. It also was windy, the wind was blowing drill dust around. A gust of wind blew a bunch of dust in the face of the driller. Some particles went around or up under EE's safety glasses and got into EE's eye.

Employee was performing maintenance on a roll away box using a come-along when the hook came loose and struck EE in the head.
EE felt a pop in EE's left knee while lifting an I-beam. Did not seek medical treatment until 6/17/2024
EE got debris in right eye. Dr visit 6/7/2024.
Employees were preparing to replace the bolts on the shaker motor. A pipe that was going to be used to tighten the bolts was leaning against the shaker. The oxygen hose for the torch hit the pipe and caused it to fall and hit employee. The employee was kneeling down and the pipe hit EE on the back of EE's head. Did not seek medical treatment until 6/5/2024.
Employee was replacing a piece of drill steel from the drill carousel, when it was placed on the drill rack below the carousel, the employee attempted to slide it off of the rack. The steel tube then fell on the employee's foot striking employee above the steel toe in the boot.
The employee was using a sledge hammer to put the pins in a bucket that they had just placed on an excavator and as EE swung the hammer EE heard a ""pop"" in EE's knee.
Employee was walking down the last flight of stairs at the scalping tower when EE's heel caught a rock causing EE to roll EE's ankle.
Operator was getting down from machine and lost footing on the next to top step and fell to the ground on their side and it tore a muscle in their leg and cracked a rib.
Employee tripped and fell while walking on the cat walk.
Employee was using the hose from the water truck to spray out the grizzly bars in the primary feeder underground. Employee was stepping down into the crusher mouth to spray underneath the grizzly bars. Employee stated that their foot slipped on the Hazemag Crusher Roll stepping down into the crusher mouth opening and it caught as EE fell.
Employee was removing a rock from the crusher with a pry bar when the bar hit the employee's right wrist causing a contusion. Employee felt okay until Tuesday 11/26/24 and then decided to go to the doctor.
Employee slipped descending a dozer and fell on the tilt cylinder, breaking two ribs. EE had surgery on 4/11/2025 due to ribs not healing properly. Was released to full duty on 8/5/2025.
EE was walking to the parts room to return tools and slipped and fell on ankle.
Employee was raising the inspection door on the secondary crusher when it slipped out of their left hand slamming onto their right hand. Resulting in a laceration/fracture to their right ring and middle finger.
Employee was checking the radiator on the F150 plant truck when the neck of the hose ruptured causing minor burns on left hand and forearm.
EE was working on a CAT 990H attempting to remove the loader arms. In the process of removing the pin between the Zbar and hydraulic cylinder, the Zbar pivoted momentarily pinching EE's left upper thigh between the Zbar and the cylinder.
Employee was descending steps when they rolled their ankle.
In the process of removing a 990 loader bucket tooth with a soft metal hammer, a fragment of the tooth superficially entered the employee's chest. In order to remove the metal fragment, surgery is required.
Employee was working on lifting spring bolts and hydraulic cylinders on 5 1/2 ft. crusher or moving toggle ends for hydraulic toggle. Sought medical treatment on 8/23/2024. Returned to work with no restrictions. Surgery scheduled for 9/9/2024.
Employee strained back while moving blocks to get rid of a portable stacker. On 5/23/2024, employee alleges while moving metal scraps, EE's old injury was reaggravated.
The employee was pulling a hose from the powder truck when EE felt a pain in their left shoulder. Prescribed physical therapy on 08/05/2024. Shoulder surgery on 11/1/2024.
Employee was cleaning the pin/locking mechanism on a skid steer bucket, when the pin retracted, the bucket came off and struck the employee's foot.
Employee stated EE had to drive on rough roads and was sore as a result of it. Later diagnosed on 5/1/24 put on restricted duty and is still being evaluated.
Employee was accessing a secondary crusher platform to inspect a crusher when a rock fell from an overhead conveyor striking the employee in the back of the head.
Employee was helping load a shot and while walking in the area the employee tripped over an emulsion hose. Employee landed on knees and sustained a scrape to one of them.
The employee was utilizing a torque multiplier tool to remove a tie rod form a hydraulic rock breaker and while releasing tension on the multiplier the ratchet that EE was using broke a tooth, skipped and quickly spun around and caught the employee's hand and caused a laceration.
Employee was attempting to remove a steel plate from the metal rack when the plate slipped out of their hands landing on their right foot.
While washing material from a plugged crusher, the material flowed into the employee's legs, causing employee to fall to the same level. Diagnosed with concussion without loss of consciousness, strain to neck muscles, and contusion on arm and scalp.
EE foot became tangled in a power cord and EE lost balance and fell. EE caught self with left hand on the table and left ring finger bent backwards against the pulverizer machine. EE then fell to knees after EE's hand took the brunt of the fall. Did not seek medical treatment until 12/15/2024
On 11/18/24, an employee was walking through the sand plant when employee slipped and hit left knee on the base of a support beam. Employee sought medical attention and was issued restrictions.
On 10/29/24, an employee was walking in the surge tunnel when EE slipped and fell. The employee sought medical attention and was diagnosed with a left elbow and left shoulder sprain. EE was issued restrictions and OTC medications.
Employee was using a T-post driver to drive 3/4"" electric metal tubing (EMT) into the ground. As EE was raising the driver, EE went too far up and the tubing came out of the driver cylinder. On the downward motion, EE missed the EMT striking EE's right index finger on the EMT while holding the driver handle. This caused an open wound and broken bone in the tip of EE's finger.
While opening a gate valve, the data plate/tag between the hand wheel and the nut, cut through the employee's glove resulting in a laceration to their right thumb. Their thumb was bandaged and employee was issued restrictions.
On 8/13/24, employee was throwing away a 20 lb scrap pump into metal waste bin when EE felt discomfort in EE's back right side. EE used 1st aid measures to help with the discomfort for 2 weeks. When the pain did not resolve EE sought medical treatment on 8/27, were diagnosed with a back strain and issued restrictions & medications.
Injured Employee was utilizing a torch to cut off two track bolts on an Excavator. As the employee finished the last bolt there was a spark causing them to pull the torch back. The torch then made contact with the right hand and burnt through the glove and burning the backside of their right hand causing a 2nd Degree Burn.
Employee was stripping the outer jacket off of motor wire to install in a pump when the Hawkbill knife EE was using slipped out of the insulation and ran across the top of EE's leg resulting in a laceration. EE sought medical attention, received sutures and is currently on restricted duty.
Employee was lifting a 2.5 gallon jug of oil when EE felt pain in their back.
Employee was seated on the inside of the cab of the loader when they caught their right middle finger between the door and the inside of the door frame.
Employee slipped from a two stepped step-ladder, leading EE's right flank to strike against the chock block holder that was on the backside of the loader bucket. This resulted in a fractured right rib.
Employee was descending the ladder of the haul truck at the end of the shift. While climbing down, EE's right foot slipped off the bottom rung, causing EE to roll ankle on the ground. Employee was diagnosed with a right ankle sprain and given restrictions.
Employee's foot slipped when EE was entering EE's pick up truck and twisted an ankle. EE is currently on restricted duty
Employee claimed that employee kneeled on a rock while employee was washing out and injured the knee.
Employee was removing a conveyor gearbox when the pipe wrench EE was using slipped causing EE to strike EE's left knee and smash EE's right thumb.
EE was reinstalling the AC belt on a pickup truck. In order to place the belt on, EE was using a wedge tool to guide the belt back on. EE was leaning on the core body/radiator and as EE went to move away from the hood, felt a pop in left rib cage. EE sought medical attention the next day and was diagnosed with sprained left rib.
On 2-16-24 employee (EE) in haul truck was being loaded. Bucket of the loader made contact with the bed of the haul truck. The shaking of the truck caused discomfort to EEs back. EE was provided first aid and returned to work with some soreness but was able to drive without discomfort. On 4-15-24 EE went to the clinic due to pain when sleeping and was issued restrictions & therapy.
While trying to roll the old drive belt off the sheave, EE's finger got caught in-between the belt and sheave pinching their right index finger.
An employee was pulling a rope to start an air compressor to get grease for the loader. The rope pulled out very fast, allowing EE's left hand to slip and strike a sign behind EE. The employee had gloves on, but the hand struck the sign on the side of the glove under the protective rubber. This resulted in a fractured metacarpal.
12-30-24 a fuel truck/lube oper was fueling excavator, slipped on grease, & fell into excavator. Point of contact of injury was left side.1-2-25 rec'd treatment w/no job restrictions/days off. 1-9-25 went to follow up w/no changes in instructions. 1-23-25 XRay showed fractured rib. EE hasn't missed any work & has no restrictions. But this makes the injury reportable.
Employee reported to supervisor on 1/4/2025 that employee allegedly hurt wrist on 12/4/2024, approximately one month prior, during a maintenance project while using a hammer. Employee sought medical treatment on 1/4/2025, and advised management of being prescribed a wrist brace and prescription.
Employee was changing a return roller with a hammer on the guard and was prying off the hammer to take out the return roller when the hammer slipped out and hit employee in the mouth chipping front tooth.
Employee reported that they were removing rocks from a catwalk in November and later alleged soreness in their left hamstring. Employee thought they would be fine, but approximately a month later, was still feeling pain in the left hamstring and sought medical attention on 12/09/2024. On 12/09/2024, management was made aware of employee being prescribed physical therapy.
Employee tripped over an uneven transition between gravel and the asphalt parking lot.
Employee was performing maintenance to the toggle system on the jaw crusher when a foreign body entered EE's eye causing irritation. EE sought medical attention on August 29th, prescribed eye drops and released to full duty.
Employee pulled away from office in service truck and was struck in passenger door by customer truck. On 12/12/2024, employee sought medical attention where management became aware EE was given physical therapy order.
The employee was in the process of preparing a wing wall on a feed hopper for rigging when the incident occurred. EE was attempting to free up a shackle that was stuck on the structure by pulling on the chain slings attached to it. The shackle sprung and hit the EE on the nose. EE was taken to local hospital where EE was evaluated, treated and released with no restrictions.
EE was welding the frame onto water tank where EE was on a ladder that was not folded out. EE was repositioning when EE lost footing and fell to the ground resulting in a laceration requiring stitches.
Employee was working overhead, changing a hydraulic hose on a water truck, when a socket tool they were using fell and struck their mouth, breaking three teeth.
Was removing silt fence, after cutting fence EE went to step away, slipped in the mud and stabbed self in wrist. EE was wearing gloves, cut was just above glove.
Employee was performing fire watch when employee received welding flash to eyes.
Employee shift started at 4PM on July 10, 2024. Incident happened July 11, 2024 at 1:25 am. Employee fell on right knee, causing a laceration, while inspecting screens and wash deck. Employee received sutures and was released to full duty.
Employee was using a pocket knife to cut a poly strap on a box when the pressure released from the strap striking the employee's leg causing a laceration.
A rock dislodged from the wall while scaling landing on miner's right hand and scale bar. Miner was wearing CUT 5 level gloves. Injury did not require sutures, but x-ray showed minor fractures of the thumb proximal phalangeal shaft.
Employee was cutting a water hose toward the barbed end of the nozzle when the knife slipped and cut the top of EE's left hand between the thumb and index finger.
Employee was trimming the bottom tab off a poly-deck screen so it would fit flush against the side of the screen. EE was utilizing a retractable razor knife and as EE cut away from self the blade slipped off catching the tip of EE's left ring finger.
Employee struck their finger with a hammer causing a minor laceration. The doctor gave the employee a splint.
1st EE & 2nd EE were welding/grinding single sand screw to mend it. 1st EE was under sand screw to weld when 2 EE began grinding causing sparks & debris to fly towards 1st EE. 1st EE removed self until 2nd EE finished. 1st EE was in full PPE; face shield safety glasses gloves hard hat & fall protection. EE reported it on 2/19. Diagnosed with conjunctivitis & prescribed eye drops.
Employee was changing the cylinder on the flop wrench and the cylinder fell on EE's finger. Employee was not wearing gloves.
Employee was welding in a transfer chute when EE overheated.
Sometime before 06/24/2024 a trespasser entered property and fell off the active highwall. Body was discovered at ~4:20PM by two EEs. MSHA hotline called at 4:27PM along with police and rescue. Sheriff's office has ruled it an accident.
