A roof fall occurred on the #6 unit supply rd. in the 1st N.W. submains. The length of the fall was 20' long x 20' wide and 10' thick. There was no equipment involved in the fall. The ventilation was not affected. no personnel involved.
A roof fall occured on the #3 unit in the 3rd NW sub panel one break outby last open x-cut in the #1 entry. Ventilation, Personnel or equipment was not affected.
A roof fall occurred on the #3 unit outby the tail piece in the #5 entry in the 3rd nw sub panel. Ventilation, personnel, nor equipment was involved. Length 20' x width 19' x thickness 6'.
A roof fall occurred on the #3 unit in the 3rd nw sub panel in the #6 entry outby the tail piece. Length of fall was 90' x width 19.5' x thickness 5'. There was no equipment involved in the fall. Ventilation was not effected nor was personnel.
1B supply Rd. 1st SW submains between x-cut 10 and 11. 60= cross cuts outby #1 unit. 45Lx20Wx7T. Ventilation, equipment and personal were not effected.
A fall of roof occurred on #6 unit in the x-cut between the #4 and #5 entry in line with the feeder. Length 50' x width 20' x thickness 5' to 8'. Ventilation, personnel, equipment was not affected.
A fall of roof occurred #6 unit #3 entry 1 x-cut behind tailpiece blocking escapeway. Escapeway was re-routed. Cribbed and timbered around. Length 120' width 19.5' thickness 5'. There were no personnel affected by the fall, ventilation was not affected nor was there any equipment affected.
Fault project: old #5 supply road at x-cut 652. Lenght of fall 20' by 19' wide by 10' thick. There were no personell affected by the fall nor was equipment or ventilation affected. This fall was on a previously installed set of arches.
A fall of the roof occurred on the old #5 unit. Fault project suppy rd. Length was 25' by 19.5' in width by 10' in thickness. Personell, equipment and ventilation was not involved. X-cut 655.
A fall of the roof occurred on #3 unit supply reach at x cut 18 of the 2nd ne panel. 20' length x 20' width x 8' high. Personell, equipment, ventilation was not involved.
A fall of the roof occurred on the northwest sub main panel, #3 unit one Break in front of the feeder. The fall measured 30 x 20 x 8-10 feet. No ventilation, equipment or personnel was affected.
A fall of the roof occurred on the 6th 54 Supply Road. The fall measured 20' L X 18' W X 6' H. No ventilation, equipment or personell was affected.
A roof fall occured on the 10A supply (roadway) at x-cut #19. The fall measured 100 ft in length, 19ft in width and 6-7 ft in ht. (No equipment or EE's were affected.) That was the 8th north east mains.
On 1/24/08 at apprx.408am, hoist at Dotiki IV experinced a malfuction with a field current regulator. MSHA was notified at approx. 419am
A fall of roof occurred between the 6th SW mains and the 6th SW Golf cart shack run around. The fall measured 50' x 20'. No equipment, personell or ventilation was affected.
A fall of the roof occurred on the 7th 54 belt line, crosscut #189. The fall measured 18' X 15' X 4 1/2'. No personell were affected.
A fall of the roof occurred in the 1st north west sue mains, spad #66116 the fall measured, 23 ft long x 20 ft wide 8 ft thick, no ventalation. equipment or personell was affected.
A fall of the roof occurred on #2 unit supply bolt, 2 crosscuts outby spad #66209, the fall measured 50 ftlx 19 ft w x 8 to 10 ft ft ht. No ventilation equipment or personell over affected.
Employee was getting off the seat when a rock fell out and hit the ATRS, then hit the employee in the left foot just below the ankle. It broke one bone in his foot.
The employee was taking a scoop off charge when he fell into the scoop canopy striking his head. The employee had bruising.
Employee was shoveling a belt line when he felt a pain in his lower back. The employee continued to work until 1/5/09 when his condition worsened and he began physical therapy.
The EE was installing a roof bolt, when he was pushing on the pin to straighten it out, he felt pain in his groin area. The EE did not start missing until 2/23/09.
Employee was standing with his hand placed on canopy of pinner when some material fell from roof hitting his left hand.
Employee was putting drill steels together, the insert was not seated. When the employee applied pressure the steels popped striking his right hand. The result of this was a fracture to the thumb and smashing the tip of his middle finger.
Employee was lifting a coupler from a Getman, felt a burning pain in his stomach. Employee continued to work. Employee went to doctor where his diagnosis was found to be a hernia. A surgery was scheduled for 11/18/2008.
Employee was standing along the side of the bolter when a piece of material pushed him into one of the trays on the bolter. The employee's jaw hit the tray causing a fracture to the jaw.
The employee was moving the miner cable over next to the rib. While he was pulling the cable over by hand he felt something pull in the back of his calf muscle. The employee did not start missing time until 9-30-08; he started missing when he got the results of his 9/25 MRI that was positive with strain to the calf muscle.
Employee was putting belt back together when he slipped straining his right knee. MRI later showed a torn meniscus. The employee did not miss any time until December 2, 2008 when he had surgery on his knee.
Employee was installing a roof bolt. A piece of material slid off of the boom striking his left forearm resulting in the employee receiving 16 stitches.
EE was rock dusting on 3rd shift when he hit his head on the mine roof slamming his neck, the EE went to the Dr for an x-ray and result was (neg) EE continued to work, his pain came back and EE went to Dr on 5/22/08 and the Dr took him off work and he returned on 5/28/08.
EE was tramming the miner to grease it, the tail of the miner was swung toward him. The EE split the cuts on the miner, pushing him into the rib with the tail. The EE was taken to have X-rays. The X-ray revealed a cracked pelvis.
The EE was setting bits on the miner. He had one bit that was hard to get out, so he was using both hands to swing the hammer to get it out. He swung really hard and hit the punch hammer to knock it out. A piece of metal broke off, coming back and striking the EE in the left eye. The piece of metal came over the top of his safety glasses that had slide down.
Employee was driving a golf cart when he struck the back corner of a mantrip causing him to hit his rib on the steering wheel resulting in a fracture to his ribs.
EE was standing near the ATRS end of the bolter. The operator gave verbal communication that he was backing up. When the bolter started to back up, the ATRS pad on the bolter pushed the EE into the rib, bruising his back and chest. EE went for an X-ray, it was negitive, he continued to work. EE is currently working restrictive duty.
While EE was drilling a hole, he lost his suction. The EE dropped his boom to work on it. The steel stuck in the roof, when he put his hand over pot, steel fell out hitting him in the hand. Result was a broken finger.
The employee was welding on a wiper when he got a piece of metal in his eye.
The employee was removing his steels from his pinner pot when a piece of material fell off the canopy hitting him in the finger. The employee received a broken finger.
The EE was clearing some rock off the miner, when his hand slipped, causing a laceration to his arm. The EE received stitches in his arm.
The employee was standing in #5 entry fiving a safety talk when a piece of material cut his left forearm. The employee received 7 stitches.
A piece of material pushed the employee's head into the boom of his equipment causing a laceration to his ear. The employee received 16 stitches.
The employee was running the right miner when some material hit his arm. He received 11 stitches in his arm.
The employee was lowering his canopy when it hung on a pin board hitting his forehead. The employee received 6 stitches.
The employee was putting two pinner steels together, when the employee put too much pressure on the steels and bowed them apart. The steels popped hitting the employee in the hand, pinching the employee's hand.
The employee was driving a pin out of a splice with a hammer when a piece of the hammer chipped off and hit him in the arm. The employee received 4 stitches in his arm.
The employee had picked up a steel to get it out of the road way. He went to throw it into a cross cut when the burr on the end of the steel cut his ring finger. The employee received 6 stitches in his finger. This happened on #7 unit.
The employee was splicing the miner cable when the belt knife slipped, causing the employee to cut his thumb. He received 5 stitches.
The employee was walking to the phone when he ran into a pipe, striking his forehead just below the bill of the hard hat. The employee received 7 stitches.
The employee was bending a pin and hit his elbow on the head of another roof bolt that was in the tray of the bolter. The employee received 7 stitches.
The employee was installing a 10' cable bolt. When he put the end of the cable bolt in the hole, he reached back to get his wrench and the end of the cable came out of the hole striking the employee in the face under the right eye. The employee received 9 stitches.
The employee was operating a miner when some material came off the miner striking the employee in the left forearm. He recieved 7 stitches.
Employee was installing belt framing when a piece of framing smashed his thumb resulting in stitches.
The employee was removing a pipe that was feeding the 31A screen bowl dryer when the pipe shifted and set down on the left index finger of the employee. The employee ended up with a broken finger and laceration. He received 10 stitches.
The employee was using a pry bar prying on something when it slipped and the end of it hit him in the head, resulting in a cut on his head. The employee received 3 stitches.
The employee was loading a belt roller on his golf cart when he dropped the roller on his right ring finger. The employee ended up with a broken finger.
The employee was getting off the seat of the bolter. Whe he rose, his right arm struck the piece of rock that was on his canopy. He received 3 stitches in his right arm.
The EE was pulling on a piece of belt, when he felt something pop in his shoulder. MRI was positive for a small tear in his AC joint.
The EE was driving a spade into the rib to hang a cable, when he struck his thumb with a hammer. The EE suffered a broken thumb.
The employee was operating a miner. When a rock that was lodged on the miner slid off striking him in the leg into above his right knee. The result of the caused the employee to received 9 stitches.
Employee was timbering as he walked down the entry he tripped and fell causing 2 laceration to his right forearm (2 lacerations on e required 6 stitches, one required 3 stitches).
The ee was cleaning his boom off. when he cut his hand on the loose rock that was on the boom. He received 4 stitches in his hand.
The employee was cleaning rock off his miner when the rock broke and cut his finger middle finger right hand. He recieved 3 stitches in his finger.
EE was loading his plate tray, he cut the wire off of the plates, when some rust went into his eye.
EE was drilling a hole for a bolt, some material came loose around the drill steel and struck his jaw, resulting in the EE receiving 4 stitches.
Employee was dropping a miner cable down off of some cable hangers. The hook on the cable hanger cut his right hand between his thumb and first finger resulting in 12 stitches.
The EE was unloading 10' pins off the bolter when he struck the head of the 10' pin with his elbow. Causing a laceration, this accident occured the #1 unit.
Employee was attempting to push a rock into the bed of a car. The rock smashed his finger resulting in 5 stitches.
Employee was installing a roof bolt when he lowered the boom down on his foot, causing a fracture is his toe (right toe)
The employee was helping splice a cable. She was cutting the cable back, when the belt knife slipped and cut her right leg. This resulted in 5 stitches.
Employee was cutting a band on 10 ft pins, when his hand slipped causing him to cut his arm on the band. Resulting in the employee receiving 5 stitches.
