Accident type, without injuries

A fall of the roof occurred at crosscut 93 in the limited intake in the South East Life of Mine Mains B in the # 7 Entry. The ventilation, personnel nor equipment were affected.

A fall of the roof occurred in the South East Life of Mine Mains C at crosscut 442 on the 10th 54 in the #4 entry. No ventilation, equipment nor personnel were affected.

The emergencey stop switch shorted out on bottom, had to trouble shoot the problem, then replace it.

A fall of the roof occurred in the South East Life of Mine C at crosscut 538 in the #3 entry. The measurements are 35 ft. in length, 20 ft. in width and 5 to 7 ft. No equipment, personnel nor ventilation was affected.

A fall of the roof occurred in the South East Mains in the # 4 entry at crosscut 181. No equipment, ventilation nor personnel were effected by this fall. This fall is more than 8 miles away from the closest unit.

A fall of the roof occurred in the SE Mains at crosscut 186 in the #3 entry on the supply road. The Measurements of the fall are 35 ft. in length X 20 ft. wide and 6 ft. high. Ventilation, passage nor personnel were affected.

A fall occurred in the SE life of mine mains C at crosscut 385 in the #3 entry. The fall measurements were 20' in length X 20' wide X 6' thick. No equipment, ventilation nor personnel were affected by this fall.

A fall of the roof occurred in the 1st SE Sub Mains x-cut 5 between #2 and #3 entries. The measurements of the fall are 55' to 65' in lenght X 19' wide X 5' to 7' high.

The following falls were discovered in th 9th SW Mains Intake in the #2 entry. From x-cut 25 to 27 it was (150ft l x 20 ft w x 5 ft to 6 ft). At x-cut 42 to 48 it was (560 ft L x 20 ft W x 5 ft to 6 ft H) and from x-cut 62 to 64 (140 L x 20 W x 5 ft to 6 ft H). This area was developed 9-30-07. This form is being submitted to abate citation # 8498443 issued on 5/6/10.

Accident type, without injuries

An unintentional roof fall occurred in the 2nd SE panel, #6 Unit,#3 Entry and inby the last open crosscut. The area of the fall included both a recently mined, unbolted cut and where roof bolts prev. had been installed. The fall stopped prior to reaching the outby section. The fall meas. approx. 70'L. X 20'W. and 12'-15' H.

A fall of the roof occurred on the 6th 54 belt at x-cut 128 in the South East Life of Mine Mains B.

Struck by falling object

The injured miner was working at the face area when an isolated portion of the mine roof fell unexpectedly. The accident occurred in an area where roof bolts previously had been installed inby the last open crosscut. The fall measured approx. 70' in length, 20' in width and 12'-15'in height. Caught in, under or between a moving and a stationary object

The employee was putting glue in his glue box opposite the operator. When the operator started to tram the bolter forward, he caught the man's foot on the oposite side between the tires of the bolter.

Absorption of radiations, caustics, toxic and noxious substances

Over pressure occurred in discharge line of shotcrete machine. Opened the joint at 2 1/2 vic clamp causing chemical burns to left side of his back and his left arm.

Over-exertion in pulling or pushing objects

The employee was pulling on a conveyor belt by hand trying to splice it back together. As he was pulling on the conveyor belt, he felt a strain in his lower back. The employee continued to work until he went off on 8/30/2010.

The employee was going to hookup two trailors when he got his foot hung in a piece of belt that was lying on the ground causing him to strain his leg and knee.

Struck by falling object

The employee was installing a roof bolt when a piece of draw rock fell out between the ATRS and the canopy of the bolter.

EE stepped on a hose fracturing his right fibula.

Struck by flying object

The employee was driving a diesel ride down the road when he ran over a rock that was lying in the road. It flipped up and hit him in the left knee. The employee continued to work and his knee became worse until he could not work anymore. The employee started missing work on 10-25-2010.

Fall to the walkway or working surface

The employee was walking around a piece of equipment when he got his foot caught in a curtain causing him to fall and land on his right wrist. The employee continued to work until 6-15-10 then he started missing work.

The employee was putting a hitch in a trailer when he felt a pain in his neck and shoulder blade. The employee continued to work until 6/8/2010 when he started missing work.

Caught in, under or between a moving and a stationary object

The employee was operating the miner attempting to cut a feeder hole. He had stopped the miner and was attempting to walk past the cutting head when one of the tram levers came in contact with a bit on the drum of the miner, resulting in the miner tram to engage and catch the operator between the drum and the rib.

The employee was operating a continuous miner when a piece of draw fell out striking him in the upper and lower back.

The employee was shoveling a belt when he caught two fingers of his right hand on a lump of coal pulling his fingers back. The employee continued to work until 4-15-2010, when his finger started to hurt him worse.

The employee was cleaning out the duct work on the miner. He opened the door on the top of the duct work to clean it out. While the door was open the employee put his hand inside the duct work, then the door fell on his hand. The employee suffered contusions to his left wrist. The employees wrist continued to hurt and he started missing work on 4-27-09.

The employee was examing the belt when he tripped on the 6"" water line causing him to hurt his lower back. The employee continued to work with pain and continued to see the doctor for it until it got to hurting him alot worse, then came out on 4-13-2010 due to the pain then the doctor took him off work.

Over-exertion in lifting objects

The employee was lifting a welder up to put in the back of a supply jeep whn he felt pain in his lower back. The employee continued to work until he got the results of his MRI back on 2-11-10, showing a bulging disk in his lower back.

Struck by falling object

While the bolter was moving up the employee was beside the bolter when a piece of material fell out from the rib and the corner of the roof hitting him in the lower back causing some hair line fracture's to his vertabra.

The employee was walking up the slope when his left foot slipped out from under him causing him to land on his right knee. The employee continued work with some pain. The pain became worse and the employee started to miss work on 10-20-2010.

While the employee was bolting a place some material fell striking him in the lower back, leaving scrapes and bruises.

The employee was running a remote control miner when some material came in contact with his right arm and shoulder.

The employee was walking beside the belt when he tripped and fell and he hit his right hand on the top belt roller.

The employee was plugging up a set of batteries on a scoop when the plug blew up on him.

The employee was looking in the reel compartment of his shuttle car with his hand on top of the reel when a piece of material fell out and landed on his arm causing a laceration.

The employee was drilling a hole when a piece of material fell out and stuck him in the upper right thigh.

The employee was taking a curtain down that was nailed up with several nails, so he had to cut it down. While the employee was cutting the curtain down the belt knife slipped and cut the employee's left middle finger.

The employee was using a hydr. jack to push a pin out of a blade on a dozer, when the jack slipped, catching the operators finger between the jack and the blade.

The employee was caring a hundred ton jack down a air course on a ride when the jack started to slide off the ride and he tried to catch it, when he caught his finger between a timber and the jack.

The employee was trying to get two pinner steels apart when the bottom one came loose and hit him just below his nose.

The employee was in the process of installing a pin, when some material fell and hit the back of his hard hat and knocked his hard hat off. Then another small piece fell, landing on the back of the employees head.

The employee was installing a cable bolt next to one that was damaged. When he put the other cable bolt in the hole and pushed it up he got his thumb caught between the two cable bolt plates and smashed the end of his thumb.

The employee was removing a rock from the conveyor belt by pulling, pushing, lifting and twisting when he felt pain in his back.

The employee was cutting a 3/4 inch hose when the belt knife went through the other side of the hose and cut the employee's finger.

Employee was starting his first steel in the roof, when a piece of gob fell out catching his ring finger on his right hand between the piece of gob and the boom.

The employee was operating a continuous miner when he twisted and his back popped.

The employee was lifting a top frame to install it when he felt a pop in his lower back when he started to hurt.

The employee was installing an 8' cable bolt when his glove got caught between the glue and the pin. When the employee jerked his glove out from being caught the pin came back and hit him in the forehead, cutting him.
