A 3/8" chain was fastened around a large rock that got jammed inside the jaw. The employees took out the slack and then he directed the loader operator to lift the rock out. As the rock was being lifted out the chain snapped and hit employee in the right side of the back. The impact knocked the wind out of him.

EE was replacing a cross member and replacing bolts in S51(Vibrating Grizzly), while he was kneeling down he felt a pop in his knee and it was hard to get up and down off the plant.

Employee was working in the screen plant welding on some guards. At a point in the process he had the helmet up and did not put it down correctly for the work he was doing. He felt he may have been in a hurry due to the fact that he had others holding the plate of steel in place while he was tacking it in.

Krause Bros Construction · Struck against a moving object

Employee was driving too fast and did not slow down properly for the embankment. He lost control. The box started bouncing around and went off the embankment which caused him to tip over causing the employee to bump his head. Employee was wearing seat belt at time of accident.
