A 3/8"" chain was fastened around a large rock that got jammed inside the jaw. The employees took out the slack and then he directed the loader operator to lift the rock out. As the rock was being lifted out the chain snapped and hit employee in the right side of the back. The impact knocked the wind out of him.

Employee was driving too fast and did not slow down properly for the embankment. He lost control. The box started bouncing around and went off the embankment which caused him to tip over causing the employee to bump his head. Employee was wearing seat belt at time of accident.
