Showing all 24Was checking out the water truck(1983 Ford 700) and was coming down the step from the cab and stepped on a small rock and twisted left ankle.
A 35D volvo dumptruck operator was climbing down from machine, stepped wrong and twisted right ankle and came to the shop and elevated and put ice pack on ankle for swelling and was taken to the emergency room for evaluation.
Employee was prying up with a bar in a man lift 65' in the air. The boom flexed downwards and when the employee released pressure EE's hand was on the top hand rail and was mashed between the elevator and man lift when the boom flexed back.
The employee wasn't feeling good. Mine operator assessed the situation and found EE was sweating but was responsive, then called 9-1-1. The EMT evaluated the employee and found the employees blood pressure to be very high and took EE to the hospital.
Employee was standing and bending over to weld in a tight area, hyperextending EE's knee. Employee sought treatment and was diagnosed with torn cartilage in left knee on 2/19/2019.
Employee climbed onto a wooden box to close a valve that was beyond his reach. The box was unstable and employee got unbalanced and fell 2 feet. He hit his right hand/index finger on the metal brace of the air filter valve he was closing causing him to cut his right index finger.
Employee bent over to pick up a 50 lb bag of siderite and hurt his lower back.
EE STACKS 50 LB BAGS EVERY DAY ALONG WITH FILLING 2,000 LB SACKS WITH PRODUCT. HE SAID HE FELT HIS WRIST POP AND SAID IT WAS SORE. SUPERVISOR SENT HIM TO THE DOCTOR THAT DAY. DOCTOR SAID IT WA S TENDINITIS.
THERE WERE NO WITNESSES TO THE INJURY. THE EE SAYS HE WAS CLEANING ROCK AND DEBRIS FROM UNDER THE CONVEYOR AND CRUSHER. HE SAID THAT SEVERAL DAYS LATER HIS BACK HURT (SAT OCT 10TH). HE REPORTE D THE PROBLEM TO THE PLANT MANAGER ON OCT 13TH, SAY DOCTOR ON OCT 14TH.
WHILE FUELING THE ROCK TRUCK, EMPLOYEE'S FOOT SLIPPED OFF THE LADDER STEP AND HE FELL BACKWARD TO THE GROUND AGAINST THE FUEL TANK SUPPORT INJURING HIS COCCYX.
THEY WERE INSTALLING A BUTANE PIPE NEAR THE ROCK BIN. HE WAS LOOKING UPWARD DURING INSTALLATION AND A PIECE OF METAL FELL IN HIS EYE.
EE WAS CHANGING A HEADLIGHT ON THE FORKLIFT. HE DROPPED HIS TOOL & ENDED UP PULLING THE MUSCLES IN HIS RIGHT SHOULDER & NECK.
EE HAD TAKEN SOME TIRE TO ATLANTA TIRE TO BE REPAIRED. IT WAS GOING TO BE ABOUT 30-40 MINUTES BEFORE THE VENDOOR COULD UNLOAD EE, SO EE DECIDED TO WAIT IN THE TRUCK. EE FELL ASLEEP WHILE WAI TING AND GOT TOO HOT.
A LARGE ROCK JAMMED IN THE SHAKER EE WAS USING A BAR TO TRY TO LOOSEN THE ROCK HIS HAND SLIPPED & STRUCK EQUIPMENT
EMPLOYEE WAS SORTING ROCK. HE SLUNG HIS HAND BACK ONTO AN ANGLE IRON. HE WAS HIT AGAINST HIS KNUCKLES.
EE WAS HELPING OTHER EE CHANGE A TIRE ON THE KENWORTH EE SLIPPED IN THE MUD AND FELT A RIPPING SENSATION IN HIS ABDOMINAL AREA.
EE WAS LIFTING A HEAVY ROCK. WHILE HE WAS LIFTING HE TWISTED AROUND REAL FAST AND STRAINED HIS LOWER BACK.
EE WAS LIFTING A LOADER ARM HE LOST HIS BALANCE & SPRAINED HIS RT ANKLE WHEN HE FELL
EE WAS PERFORMING HIS REGULAR WORK TASKS WHEN HE ACCIDENTALLY RAN HIS HAND OVER AGAINST A RUST NAIL. HE WAS SENT TO THE DR AS A PRECAUTION.
EMPLOYEE BEGAN BEING ABSENT FROM WORK ON 11/2/93STATING THAT HE WAS SEEING A DOCTOR FOR SOME NONWORK RELATED BACK PROBLEMS THAT HE WAS EXPERIENCING. THE DOCTOR SEEMS TO THINK THAT HIS PROBLEM IS WORK RELATED AND THAT THIS SITUATION SHOULD BE HANDLED BY THE TEXAS WORKERS COMPENSATION.
EE REPORTS THAT HE TURNED WRONG AND BEGAN TO EXPERIENCE PAIN IN HIS LOWER BACK. ALTHOUGH THE EMPLOYEE DID NOT THINK HIS PROBLEM WZS WORK RELATED, THE DR. HE SAW APPARENTLY DID THINK IT WAS WOR K RELATED.
EMPLOYEE WAS SCRAPING ROCK WHEN DEBRIS FLEW INTO HIS LEFT EYE. HE DID HAVE ON HIS SAFETY GLASSESAT THE TIME FO THE INCIDENT, HOWEVER THE DEBRIS WENT UNDER THEM AND INTO HIS EYE.
EMPLOYEE WAS CHECKING THE LEVEL OF ROCK IN THE ROCK BIN WHEN HE SLIPPED AND FELL FROM THE ROOF TO THE GROUND APPROX. 10 FEET. ROOF AND LADDER WERE DRY. HE APPARENTLY RECEIVED A HAIRLINE FRACTU RE TO ONE OS HIS RIB BONES WHERE IT CONNECTS TO THE SPINE.
