A 35D volvo dumptruck operator was climbing down from machine, stepped wrong and twisted right ankle and came to the shop and elevated and put ice pack on ankle for swelling and was taken to the emergency room for evaluation.

Employee was prying up with a bar in a man lift 65' in the air. The boom flexed downwards and when the employee released pressure EE's hand was on the top hand rail and was mashed between the elevator and man lift when the boom flexed back.

The employee wasn't feeling good. Mine operator assessed the situation and found EE was sweating but was responsive, then called 9-1-1. The EMT evaluated the employee and found the employees blood pressure to be very high and took EE to the hospital.

Employee climbed onto a wooden box to close a valve that was beyond his reach. The box was unstable and employee got unbalanced and fell 2 feet. He hit his right hand/index finger on the metal brace of the air filter valve he was closing causing him to cut his right index finger.
