Employee was taking a ph sample in the Redox tank, when he opened the lid to get the sample and was exposed to chlorine vapors. He was given oxygen on site and then transported to a physician for observation

On 5/20/09 employee complained of tearing and discomfort in left eye, upon observation a foreign particle was observed in his eye and he was transported to a physician for medical treatment. The employee stated that he was not aware of an incident that could have caused it but thought that it might have occurred when cutting and threading pipe the day before.

Employee was pressure washing the plates on a filter press and attempted to twist the nozzle on the end of the sprayer to adjust the spray pattern. While twisting the nozzle, his little finger slipped off of the end of the spray gun into the high pressure spray. This resulted in a small laceration to the employees right little finger
