Showing all 7While exiting cab of truck, driver fell from top step. Driver complained of pain to EE's left arm. This driver was not an employee or contractor of Baserock Minerals. This driver worked for Strack Excavating and was our customer.
Employee was operating a skid-steer at the plant. The employee's operation of the controls caused the skid-steer to abruptly change in speed, causing the employee to be jolted inside the cab. The sudden jolt caused the employee to feel pain in middle back and left shoulder.
While employee was treating for elbow injury, doctor discovered carpel tunnel and added new treatment program which included surgery.
The jaw crusher was plugged with rock. Employees were removing smaller rocks by hand. EE lifted a rock and their elbow popped and began hurting.
Stepped on a 3 inch rock, twisted ankle.
Was exiting loader climbing down ladder and landed on twisted ankle.
