Stepped on a 3 inch rock, twisted ankle.

Was exiting loader climbing down ladder and landed on twisted ankle.

Employee was attempting to unclog crusher. As he was getting up into a standing position, he braced himself by placing his hand in access window. A rock dislodged and fell on his hand/finger.

The door latch was stuck. He pulled the latch and strained his arm.

He was cutting a belt and cut his finger

Changing the oxygen tank. It slipped out of his hands and when he grabbed it while it was falling, he strained his lower abdomen.

Using prybar to free clogged material in the shoot on top of cone crusher. Boulder broke free while using prybar causing boulder to smash finger between bar and hopper.

Employee slipped while descending from a temporary scaffolding system and fell about 3 feet to the working platform. Bruised Right shoulder and Right side of neck.

Descending ladder of 500 Komatsu and pulled a muscle in his rib.

Stepped off ladder on Tyler Screen and fell on the rocks.

HAUL TRUCK AND LOADER BACKED INTO EACH OTHER.
