Showing all 11On Saturday, July 21, 2007, the victim, while trespassing on mine property, was operating a 4-wheel recreational vehicle and traveling on the mine haul road. He lost control of the vehicle resulting in serious injuries. The victim died of his injuries on July 25, 2007.
The retail tractor trailer's load of ash stuck in the trailer as it was dumpping its load and cause a unbalance to the trailer which rolled onto its side. The truck was over loaded by 6440 pounds.

EMPLOYEE WAS STANDING ON A LADDER TRYING TO DISLODGE FROZEN MATERIAL FROM CONVEYOR. LADDER SLIPPED CAUSING EMPLOYEE TO STRIKE EDGE OF CONVEYOR FRAME WITH HIS MID TORSO.
COLD WEATHER, FROST. EMPLOYEE WAS HANDING FUEL HOSE UP TO THE 4600 DRAGLINE OPERATOR TO COMMENCE FUELING. THE FROST COVERED NOZZLE SLIPPED OUT OF THE OPERATOR'S HAND. THE NOZZLE KNOCKED OFF HA RD HAT AND NOZZLE HIT EMPLOYEE IN THE HEAD.
EMPLOYEE WAS PUTTING DOWN LADDER. THE LADDER STARTED TO FALL & EMPLOYEE GRABBED IT AND PULLED MUSCLES IN HIS SHOULDER AND NECK.
WHILE EMPLOYEE WAS EXITING OPERATIONS CAB OF #34 DRAGLINE HE SLIPPED ON THE STEPS AND HIT HIS RIGHT ELBOW. NO MEDICAL TREATMENT REQUIRED. WE WERE INFORMED ON 6/19/03 THAT FLUID STARTED TO ACCU MULATE AND ON 6/24/03 EMPLOYEE WENT FOR MEDICAL TREATMENT.
OPERATING D8R #1 DOZER CONTROLS. NOTE: RECEIVED A DOCTOR'S DIAGNOSIS ON 4-22-03 THAT SAID THE EMPLOYEE HAS CARPEL TUNNEL SYNDROME OF BOTH HANDS.
