While ascending Cat D10T bulldozer, Asset #DZ032, serviceman lost footing due to muddy boots, slipped and LEFT rib area hit steel push arm causing soreness. Employee was taken to local Urgent Care for observation and diagnosed with LEFT 9th lateral rib fracture.

Employee was performing maintenance on Volvo A40D, asset #TR403, and dropped belly pan. While working to assess for repair, bolt broke that holds clip securing safety chain of belly pan causing metal to impact LEFT index finger. Employee was taken to local Urgent Care and diagnosed with laceration to LEFT index finger and received two sutures.

Employee called off work 3/05/24 stating ""last night I had piece of metal removed from eye, I won't be in."" On 3/06/24 met w/ employee and employee stated on 3/01/24 while performing maintenance of 390 bucket they felt irritation/soreness to RIGHT eye. Employee was taken to local Urgent Care for observation, sent them to eye doctor and diagnosed with foreign body to RIGHT eye.

Performing pre-operational inspection employee attempted to clean off rear reverse light on asset #TR602 and received a cut to LEFT middle finger, applied bandage and finished shift. Next day employee went to Urgent Care for observation, diagnosed with laceration to LEFT middle finger and received three stitches.

While performing maintenance pressure washing hydraulic coolers on Cat 6030 employee squeezed trigger, wand pushed back allowing nozzle to brush across RIGHT hand causing pain to RIGHT finger area. Employee removed gloves and noticed bleeding and was taken to local hospital for observation and diagnosed with RIGHT 4th finger laceration and placed on restrictions.

While talking on phone to another mechanic, employee turned to begin walking back to fuel truck and feet slipped causing employee to fall to ground and felt pain in right foot and ankle area. Employee was taken to local Urgent Care and diagnosed with right ankle sprain.

While performing maintenance pressure washing the 6030 SH002, employee slipped and lost balance causing fall to ground. Employee extended arms attempting to brake fall and felt pain in left wrist after impact with ground. The next day employee was taken to local Urgent Care and diagnosed with possible fracture and scheduled for MRI.

While performing maintenance of JCB 940-4 Forklift, employee leaned forward against machine and right knee pressed against metal frame. Knee hyperextended backwards causing pain and soreness to right knee area. Employee was taken to local Urgent Care for observation and diagnosed with pain in knee and possible tendinopathy of patellar tendon.

While descending ladder on dragline DL039, employee lost footing and took right arm to grab backwards to stabilize self and felt pain in right shoulder. On 2/26/18 employee refused medical attention. On 4/23/18 employee was taken to local Urgent Care for observation and put on restricted duty.

Operator of 980H loader was observed slumped over in machine. Employees removed operator from cab, placed on ground, began CPR after finding no response and contacted local EMS. Paramedics removed employee from mine site, took to local hospital for further observation. Employee was pronounced dead at hospital. Coroner report identified cause of death as massive heart attack.

While placing chain onto metal tooth of loader bucket, employee's right hand was pinched between chain and metal tooth. Employee was taken to local hospital for observation. Employee received thirteen stitches to right middle finger and diagnosed with finger laceration.

While performing maintenance changing steel hydraulic tube, employee loosened bolts and hydraulic oil splashed EE's face and eyes. Employee was taken to Urgent Care for observation and diagnosed with superficial chemical burn of both eyes.

While operating Cat D9L bulldozer in reverse, machine hit rock causing employee's left elbow to bang against metal frame inside cab. On Tuesday March 28, 2017 employee was taken to local Urgent Care for observation.

While shifting weight onto right leg, employee felt pain and soreness to right knee area and visited local hospital for observation. Employee was diagnosed with a sprained right knee w/ possible MCL damaged.

Employee sprained left wrist changing wire rope on dragline 11/02/16 and declined medical observation. On 11/15/16, employee reported the left wrist is still sore. Employee was taken to local Urgent Care for medical observation and diagnosed with a sprain to left wrist and put on restricted duty.

While performing maintenance on Cat 6030, Asset #SH002, employee tightened hose clamp on left engine coolant hose. Hose separated from metal fitting causing hot antifreeze and steam to hit left wrist area. Employee was taken to local Urgent Care for observation and diagnosed with 2nd degree burn to left wrist area.

While re-mounting haul truck, employee attempted to pull self up while holding onto ladder, EE felt pain and soreness to EE's left shoulder area. Employee was taken for initial observation to local Urgent Care and diagnosed with strained muscle.

Employee was swinging sledge hammer attempting to hit a 4"" x 68"" piece of steel to break off full sheet. After swinging sledge hammer, employee felt pain and soreness to right rib cage area. Employee was taken to local Urgent Care and diagnosed with a sprained muscle to right rib area.

Employee attempted to flip a steel fender into a loader bucket. After letting go, the steel fender fell backwards hitting employee in the right knee area causing a laceration. Employee was transported to local hospital, diagnosed with laceration, and received 15 stitches.

While operating empty haul truck, employee drove over bump in road entering shovel cut that jarred lower back section causing soreness and pain to lower back area. Two days later on 9/10/15, employee was still sore and taken for observation and diagnosed with lumbar strain.

Employee attempted to descend the stairway on 4600 dragline, lost his balance and slipped, causing a fall forward and his chest area impacted the steel hand railing causing pain to chest/left side area. Employee refused medical attention. However, three days later on 12/13/14 employee was still sore, went to local hospital for observation and diagnosed with one fractured rib.

Employee was making repairs, grinding on shovel bucket SH002, to install new wear bars. At home later that evening, employee noticed a piece of metal in his left eye. On 9/22/14, the next workday, employee was taken to hospital for observation, diagnosed as having foreign body in left eye, sent to eye doctor. Eye doctor removed a metallic foreign body from the left eye.

Employee was walking, carrying fuel nozzle and dragging hose when his feet got stuck in mud causing him to lose balance. Employee fell forward and his right knee impacted the ground causing pain and soreness. He was observed at the local hospital and diagnosed with a knee contusion.

Employee was walking around the back of his personal truck to retrieve a tool, slipped on ice causing a slip/fall to the ground causing injury and bruise to right shoulder area.

While attempting to lift and secure the crane load block on the service vehicle, I felt a strain in my left stomach area.

While attempting to descend the steps on the RH120E excavator, employee slipped and fell then landed on the metal platform outside the engine room. Employee suffered a slight concussion and a bruise to his upper back area.

While standing on wooden ladder and performing maintenance on the steel hopper at the tail section of the McCloskey wheeled stacking conveyor, the ladder slid causing employee to twist, and while grabbing hold of the steel hopper this allowed his left leg to strike the steel hopper, causing a contusion to his left leg in the area of his thigh.

While attempting to lift metal screens, employee felt a pain in stomach area. Not sure what it was, employee went to see Doctor on his own. After reading cat scan, employee was diagnosed with a hernia. Employee will have operation at a future date.

While handling and unloading metal screens without PPE (gloves), employee reached to grab the next screen and received a laceration to the palm area of his right hand. Employee received 4 sutures.

While performing maintenance employee suffered an insect bite to his left forearm. After wound became red and swollen, employee went to hospital for observation. He was diagnosed with MRSA staph infection and kept in hospital to treat infection. Employee has not yet returned to work.

A 15 year old juvenile fell and injured his left ankle while tresspassing on an inactive part of the mine site.

Employee was operating haul truck TR604 and started experiencing chest pains. After parking haul truck employee descended haul truck, called foreman, and then taken to local hospital for observation.

While attempting to close the operator's cab door on the H285S Hydraulic Shovel employee pinched his left index finger on the door hinge.

Engineer was climbing on waste pile while surveying and when he went to turn around to look at the last set point, he felt a tear or stretch on the outer part of his left knee, causing discomfort.
