EE is a participant in Delta's hearing conservation program & has been informed annually for many years of hearing loss. EE has refused to use proper hearing protection or follow-up w/ further medical evaluation. In addition, EE operates heavy farming eq., logging eq., & chainsaws outside of work environment. The cause of hearing loss cannot be attributed directly to mining work.

HURT SHOULDER WHILE CHANING 110 LB DRILL STEEL. HE WAS PUT ON RESTRICTED DUTY TO SEE IF NOT USING THE SHOULDER WOULD HELP. EE DID HAVE A PREVIOUS SHOULDER INJURY PRIOR TO 1997. AFTER MRI SAID NO WORK FOR 2 WEEKS.
