Caught in, under or between collapsing material or buildings

A miner died when a highwall collapsed on the excavator the miner was operating.

Struck by falling object

EE's working to clean the mainframe arm on primary crusher. During the lift, the rigging assembly appears to have failed, striking the EE who was located in the line of fire.

Fall from machine

EE DID NOT UTILIZE LOTO OR FALL PROTECTION. PER VIDEO & EYEWITNESS, EE WAS WORKING ON ELECTROMAGNET ABOVE CONVEYOR BELT #2. EE STEPPED ON THE BELT FOR THE ELECTROMAGNET THAT TURNS ON EVERY 15 MIN FOR A FEW REVOLUTIONS. THE BELT ENERGIZED AROUND THE TIME EE STEPPED ON IT & THE BELT SWEPT EE OFF THE TOP OF THE ELECTROMAGNET. EE FELL APROX. 20-25 FT STRIKING HEAD & OTHER BODY PARTS.

Caught in, under or between collapsing material or buildings

The mine slope collapsed trapping our miner in his machine. The recovery effort was not successful and the employee expired at the mine site.

Contact with electrical current

Employee of Adams Trucking was weedwacking brush. Blade on weedwacker hit guide wire for electrical pole. The wire was severed and the tension pulled it back toward the transformers on the pole. the guide wire came in contact with a bare wire. At this time we assume electricity went to ground causing fatality. Under Investigation.
