EE was exerting force on a jack in an effort to reposition a rail car. While operating the jack, He felt a pain in his groin area.

Employee was stacking fifty pound bags of product into a presswell when he felt soreness in his left elbow.

Employee was walking down the stairs at #3 Auto Packer. His right hand slipped off the railing causing him to strike his arm against the hand rail. This case became reportable when he was placed on limited work activities on 5/17/05.

EE was using a jack hammer to break some concrete when his left wrist became sore. The doctor placed EE on restricted work activities.

EE was using a hammer to align a pipe flange. He struck the tip of his right thumb with a hammer blow that inadvertently missed the pipe flange. This case became reportable when Max was seen by a physician on 4/19/05 and an x-ray revealed a fracture.

EE was packing product at the 10# packing station. When She pulled a full bag of product off, the packer blew out some air and product and She sustained a foreign body in the right eye.

WHILE REMOVING A SECTION OF HANDRAIL, EE WAS HOLDING HANDRAIL AND PULLING IT UPWARD WHILE OTHER EE WAS STRIKING HANDRAIL. EE THEN MOVED HAND AND OTHER EE STRUCK HAND WITH PRY BAR.

EE was walking along the preswell area of #6 Air Sifter packaging station when he tripped on the floor mat.

On 2/21/05 at approximately 8:00 am, EE was using a shovel to dig a trench. When pushing on the shovel, EE felt pain in his right thigh. This case became reportable when EE requested medical attention on 2/28/05 for the discomfort in his right thigh.

EE closed some bottom dump crude car doors manually on 2/13/05 at 4:30 pm when he felt pain in his back. The door closer was malfunctioning again on 2/17/05 and EE manually attempted to close the car doors when he felt pain in his low back. EE was treated and placed on modified work duties on 2/17/05.

EE WAS CHECKING ELECTRICAL WIRING ON #8680 LOADER. EE WAS STANDING ON TIRE OF #8680. SLIPPED AND CAUGHT/BROKE FALL WITH RIGHT HAND. HAND STARTED TO HURT. TIRE HAD SNOW ON IT CAUSING EE TO SLIP.

Employee was cutting shrink film off a pallet of bagged product. EE was cutting in a downward motion with his left (dominant) hand while holding the film with his right hand. The knife blade came in contact with his right index finger at the middle knuckle, causing a laceration.

EE was packing 50 lb bags at parking station a bag blew up, causing dirt to get in his right eye. EE was wearing safety glasses at the time of the accident.

Employee was mounting elevator buckets from floor to other employee who was installing them. Employee went to doctor on 12-9-04 for strained back.

Celite feels this is not a valid injury, employee not working, Supervisor stated injury occurred at home. Claim denied by L&I. New claim using injury date 11-30-04 accepted 1-28-05 by L&I. Acc rpt filled out 1-18-05 stated he pulled pallet from stack set on machinery for supersack bags felt pulling back of leg, pulled another pallet from stack felt stinging lower back.

Employee went to the Shipping break room to take break. He sat on the end of an aluminum bench. Because there was no other weight on the bench, it tipped and flipped up, causing him to fall on the floor. He sustained injury to his lower back.

Injured and another employee were removing an air line from a compressor. The air line was under pressure when the fittings were released. The pressurized hose struck him in the left hand. His work was temporarily modified to restrict use of the left hand.

ANOTHER EMPLOYEE HAD GOTTEN A LIFT TRUCK STUCK AND SECURED INJURED EMPLOYEE'S ASSISTANCE IN GETTING IT "UNSTUCK". THEY ATTACHED A TOW CHAIN AND ATTEMPTED TO PULL OUT THE OTHER LIFT TRUCK. ON THE 4-5 ATTEMPT, A LINK IN THE CHAIN BROKE AND THE CHAIN WHIPPED AND HIT EE IN THE FACE.

EMPLOYEE WAS REPLACING #2 MICROSIZER MOTOR. HE HAD LOWERED OLD UNIT THROUGH A HOLE CREATED IN THE FLOOR FOR THIS PURPOSE. HE LEFT THE AREA TO SECURE PARTS/TOOLS, REENTERED THE AREA TO GET HIS TOOLS AND STEPPED THROUGH THE HOLE IN THE FLOOR.

Employee was working in the booth at #3 Auto Packer. When employee stepped out of the booth, he stepped on a can of spray lubricant located on the floor, causing him to slip. Apparently, the can of lubricant had fallen from a storage table to the floor and he did not see the can laying in his path.

Employee was shoveling crude spillage from the floor under #2 Shaker when he felt pain in his back.

Employee was filling forklift propane bottles when he lifted a bottle and turned, twisting his back.

Employee was drilling a hole to mount a mirror on the cab of the loader and a piece of material embedded in the left eye.

EE was called to inspect an overheating forklift. After EE lifted the engine compartment canopy or hood the radiator overflowed. Hot coolant contacted EE on the right forearm, causing a burn to the skin.

Ee was hammering on a plugged vacuum pipe when he felt pain and soreness in his right elbow. This event occurred on 8/12/04 and became reportable when ee was treated by a physician and placed on modified work activities on 9/17/04.

Ee was walking to pick up samples at #5 & 6 Bulk Bin. Ee stepped in a puddle of water and slipped.

EE was working on #601 Cyclone Top Hat. EE struck a drift with a hammer. The hammer glanced off the drift, causing his right index finger to be caught between the hammer handle and the floor.

EMPLOYEE WAS INSTALLING A CONVEYOR BELT ROLLER WHILE LYING ON HIS BACK. HE WAS HOLDING A HEAVY ROLLER WHEN THE WEIGHT SHIFTED CAUSING HIS BACK TO JERK. EMPLOYEE WENT TO THE DOCTOR ON 9/10/04.

EMPLOYEE WAS CUTTING & WELDING ON A TRAILER SPRING HANGER WEAR PLATE WHEN A PIECE OF SLAG FELL INTO RT. EYE.

Wind blew perlite in right eye.

EE INADVERTENLY CAUGHT FOOT ON DECK OF YARD HOSTLER & FELL

EE was unplugging the discharge spout of a packaging machine. EE was struck in the right palm by a rod he was using to clean the a plugged packer spout.

EE was removing a blower liner from #501 blower. When the bolts holding the liner in place were removed, the liner broke in two. In an attempt to keep the ten pound section of liner from falling onto his body, he pushed at the broken piece of liner with his left hand, causing a bruise to his left thumb.

EE was operating a lift truck. He hit a wet spot, the truck slid slightly and he stuck out his left arm to keep from hitting a metal powder pump. His left hand was caught between the pump and the fork lift resulting in a crush injury.

EE complained of discomfort in his right shoulder after repeated use of a chain block to install the back head of 625A blower. This event occurred on 6/21/04 at 3:00 pm but become reportable when the treating physician placed EE on work limitations beginning 7/7/04.

Employee was using a shoe knife to cut some duct tape. The knife slipped, resulting in a laceration to Gilbert's left index finger.

EMPLOYEE WAS REPLACING SCREENS IN SCREEN DECK WHEN VIBRATION FROM AIR IMPACT TOOL THREW PERLITE IN LEFT EYE.

CRUDE ORE HAD SPILLED OFF N302 BELT ONTO FLOOR IN BASEMENT. EMPLOYEE WAS SHOVELING ORE FROM FLOOR BACK ONTO BELT AND FELT A SHARP PAIN IN HIS LOWER RIGHT ABDOMEN AREA (ABDOMEN STRAIN). EE DID NOT THINK IT WAS SERIOUS AND WANTED TO WAIT TO SEE IF IT WOULD GET BETTER BEFORE GOING TO DOCTOR. EE WENT TO DOCTOR ON JUNE 18, 2004.

Michael was using a hoist at 3 A/P to lift plastic wrap. His left thumb was caught between the hoist and an air cylinder.

While unloading truck, the wind blew perlite in left eye. Physician removed foreign object from eye.

EE sustained a back injury on 3/29/04 when lifting and stacking some blower liners. The case became reportable when the treating physician placed EE on modified work duties beginning 5/4/04.

EE stepped off a screw housing to the floor with his left leg extended. His ankle "rolled over" causing his left leg to buckle under his weight.

EE HAD PUT A SUPER SACK BAG ON MACHINE & STARTED TO FILL BAG WITH SLOWLY FALLING PRODUCT. HE TURNED AROUND TO PICK A A CARD TO INSERT IN SIDE OF BAG. WHEN HE TURNED BACK AROUND THE PRODUCT WAS FALLING RAPIDLY IN BAG. EE GRABBED BAG TO CENTER ON PALLET BUT BAG WAS TOO HEAVY TO MOVE, STRAINING HIS LEFT SHOULDER.

EE was rebuilding screw for fines system. Pulling on a 1 1/4" wrench felt pain in right arm & shoulder.*Note:EE was on vacation starting 7/19, we didn't know he went to DR (7-19-04) until he returned to his regular scheduled shift July 29th bringing paperwork from dr for light duty work. Tendinitis right shoulder-possible rotator cuff.

Employee was welding. Hardface material was cooling off; reacted cracked causing metal pieces to project from weld. A piece was on employee when he washed his face and fell into his left eye.

EE WAS BURNED ON THE LEFT WRIST FROM SLAG CREATED BY THE USE OF A CUTTING TORCH TO BURN OFF A BEARING AT #3 SECONDARY BLOWER. THE EVENT OCCURED ON 3/9/04 AND BECAME REPORTABLE 3/15/04

EE WAS WALKING IN THE CRUSHING PLANT WHEN HE TRIPPED. USING HIS HAND TO CATCH HIS BALANCE, HE STRUCK HIS SMALL LEFT FINGER ON A PIECE OF ANGLE IRON.

Employee was installing a probe on a bulk truck hatch. The probe slipped from his grasp, smashing the left thumb against bulk truck hatch.

EE WAS MOVING A HOT BLOWER FLANGE TO THE JIG. THE FLANGE FELL OFF THE JIG. HE JERKED WHEN PICKING UP THE FLANGE.

EE WAS PARALLEL TO EDGE OF BENCH AND WAS TOO CLOSE TO EDGE. ONE SET OF TRACKS DROPPED CAUSING DOZER TO FALL AND ROLL 1.25 TIMES OFF A 60 FOOT HIGH BENCH. DOZER LANDED ON SIDE. OPERATOR BOUNCED AROUND IN CAB.
