2 employees were working on a palletizer, but only one of them had it locked out. When the locked out employee was done, they asked the other employee if they were clear, and heard ""yes"". The other employee was still working, and when the machine was started, their right hand was caught, amputating the index finger, and degloving the middle finger.

IW was beginning to disassemble floating gearbox for a screw conveyor. The IW removed guarding and was in the process of removing the drive belts between the motor and gearbox. As the employee loosened the belt tension bolts, the gearbox and motor fell. As it fell, the employee's right hand and arm were pulled downward, straining the employees right shoulder.

Haul truck driver fell backward while exiting cab of truck, striking head on pavement and sustaining a 1"" cut that required staples. Employee admitted to rushing and not having mind on task, and was not certain that EE maintained 3 points of contact.
Body position obscured line of sight while lifting rail car vent lid, resulting in trip over cam locks on top of rail car. Trip resulted in partial fall into open hole. 2 ribs fractured.

The maintenance employee was trying to remove the gear box with a pipe wrench. The pipe wrench was adjusted on the collar of the gear box. The employee applied all his body weight on the handle. The jaws of the pipe wrench slip. He lost balance and fell on the shaft, head first. The employee cut his head above the right eyebrow.
The maintenance employee tried to get up from a laying position in a very cramped space and his hip popped, causing a lot of pain and stiffness

Employee was working on a separator bearing column. He reached down to pick up and move the column when he felt a pain in his right wrist. Employee did not use proper lifting techniques, resulting in a sprain to his wrist.
Employees were working on the layer pusher portion of the palletizer when one employee was caught between the layer pusher and a steel beam.
Employee was working on packaging stand and picked up a pallet at waist height to set on the floor. He started to feel pain in the groin area and it slowly got worse as the shift went on.
Employee was adjusting Super Sack Bag onto pallet as bag was filling, felt a pain in his middle/upper back.
While replacing and adjusting v-belts on C-310 Fan, the last joint of the middle finger of the left hand was nearly amputated when the hand was caught between the v-belts and the belt pulley on the drive motor.

Employee finished loading truck at dock. Was in the process of resetting loading ramp (east side). When employee started to pull chain to readjust loading ramp his foot slipped on slick metal surface causing employee to fall to floor injuring left shoulder.

Employee was filling Semi-Bulk Bags, pulled on Semi-Bulk Bag to align on pallet and felt a twinge behind left shoulder blade. Pain increased in left shoulder blade area over several hours resulting in Left Strain to Shoulder.
Employee was inside railing on top of bulk rail car and had unhooked second bulk hose to put away. Employee caught the inside of hatch cover ring with right foot, causing him to partially fall in hole, scrapping right inner thigh, elbow and twisting left knee.

Employee was on knees vacuuming floor near north packing stand door. Another employee pushed door open on his way out of packing stand, at that moment Employee vacuuming reached up and put his right hand on door jam, as door closed it caught the end of his right ring finger. Employee jerked hand back and a portion of finger plus nail was torn off.
Employee transferring from manlift to Deck #2 lost balance and landed hard on left foot while twisting felt left knee pop - pain was minor. No witnesses, accident not reported until Oct 7 to supervisor. Employee went to doctor on October 12 due to knee still popping when climbing stairs and kneeling.
While raising a demister section using a chain hoist, employee struck his gloved hand against the demister section, which has sharp metal edges, cutting his right ring finger, requiring sutures.
Installing Air Lines - EE was inside Manlift Basket, knelt down to reach pipe that was being installed, felt something tear in right knee.

Initial accident reported to Supervisor on 6-25-09, EE thought the injury was very minor and did not want to go to the doctor. The injury was still bothering him and he went to doctor on 8-17-09. He was riding railcar to switch tracks, he jumped off before car came to a stop at switch, on landing he felt a sharp pain in the back of right foot and heel areas.

The day before accident Quincy area received approximately 3 inches of snow. As the employee was walking from maintenance shop to the control room using a paved roadway on south side of plant, he slipped and fell twisting his right leg causing a fracture of the right femur.
Employee was unplugging G303 Screen deck, after unplugging screen employee was exiting from deck and caught his left ring finger between the drive shaft and a support spring. Resulting in a fracture of his left ring finger in first joint area.

CRUDE ORE HAD SPILLED OFF N302 BELT ONTO FLOOR IN BASEMENT. EMPLOYEE WAS SHOVELING ORE FROM FLOOR BACK ONTO BELT AND FELT A SHARP PAIN IN HIS LOWER RIGHT ABDOMEN AREA (ABDOMEN STRAIN). EE DID NOT THINK IT WAS SERIOUS AND WANTED TO WAIT TO SEE IF IT WOULD GET BETTER BEFORE GOING TO DOCTOR. EE WENT TO DOCTOR ON JUNE 18, 2004.
EE HAD PUT A SUPER SACK BAG ON MACHINE & STARTED TO FILL BAG WITH SLOWLY FALLING PRODUCT. HE TURNED AROUND TO PICK A A CARD TO INSERT IN SIDE OF BAG. WHEN HE TURNED BACK AROUND THE PRODUCT WAS FALLING RAPIDLY IN BAG. EE GRABBED BAG TO CENTER ON PALLET BUT BAG WAS TOO HEAVY TO MOVE, STRAINING HIS LEFT SHOULDER.
EE was rebuilding screw for fines system. Pulling on a 1 1/4"" wrench felt pain in right arm & shoulder.*Note:EE was on vacation starting 7/19, we didn't know he went to DR (7-19-04) until he returned to his regular scheduled shift July 29th bringing paperwork from dr for light duty work. Tendinitis right shoulder-possible rotator cuff.

REMOVAL AND CARRYING ELECTRIC MOTOR WT APPROX. 70 LBS TO PUT ON PALLET TO TRANSPORT TO MAINT. SHOP EE DID NOT SEE WHERE EDGE OF CONCRETE PAD ENDED, STEPPING ON EDGE WITH RIGHT FOOT, CAUSING RI GHT FOOT TO ROLL SIDEWAYS FRACTURING ANKLE. HE SAID HE HAS TROUBLE WITH BIFOCALS JUDING DEPTH PERCEPTION.
PACKER/LOADER WAS INSERTING ADQ BAGS ONTO SPOUT ""B"" IN PACKER - BAGS FILL AUROMATICALLY. THEN P/L PULLS FULL ADQ BAG OFF OF SPOUT AND BAG DROPS TO CONVEYOR BELT. ADQ BAGS DO NOT HAVE A PLASTIC INSERT IN SPOUT, MAKING IT DIFFICULT TO PULL BAG OFF SPOUT - MUST EXERT MORE PRESSURE.

EMPLOYEE WAS PRYING A JAMMED WOOD PALLET ON THE SUPER SACK CONVEYOR LINE. HE PULLED MUSCLES ON HIS RIB CAGE.
THE EMPLOYEE WAS MOVING A FEEDER HOSE FROM ONE COMPARTMENT TO ANOTHER ON TOP OF A RAIL CAR. THE WALKWAY HAD BEEN DAMAGED AND THERE WAS A LARGE DIP IN IT THE EMPLOYEE STEP OFF THE LANDING AND I N TO THE DIP AND TWISTED HIS ANKLE.
THE CONTROL ROOM OPERATOR WAS MOVING A FILL HOSE FROM THE SODA ASH CAR.
IT HAD BEEN SNOWING FOR ABOUT 4 HR AND THINGS WERE SLICK. WE HAD THE EE CLEANING OFF ONE OF THE RAMPS THAT WENT INTO THE SUPER SACK ROOM AND HE SLIPPED AND LANDED ON HIS TAILBONE. HIS BOOTS HA D SMOOTH LEATHER SOLES. THIS PLAYED INTO THE SLIP.
EE WAS WALKING BETWEEN C-302 FAN AND THE KILN WHEN HE SLIPPED IN A WET MUDDY SPOT ON THE GROUND. THE AREA HAD A LAYER OF MUD LEFT OVER FROM THE SNOW MIX WITH THE DUST FROM THE PLANT.
FROM WHAT THE EE IS SAY THAT OVER TIME HIS LIFT SHOULDER WAS HURTING. THIS EE WAS OFF WORK FOR OVER 30 DAY BEFORE HE EVER FILED A CLAIM. THE LAST DAY THE EE WORKED HE WAS OK AND HAD NO COMPLAI NTS.

THE EMPLOYEE WAS COUNTING STACK OF BAGS OF PRODUCT IN THE WAREHOUSE. HE CLIMBED ON TOP OF A STACK AND TWISTED HIS LOWER BACK. THE EMPLOYEE DID NOT REPORT THIS TILL 11/6/00.
OPERATOR II TWISTED HIS ANKLE IN THE SUPER SACK ROOM WHEN HE WAS TAKING SAMPLES, DUE TO THERE BEING NO PLACE TO PUT A REMOVABLE CONVEYOR STAND, THE OPERATOR TRIP OR THE STAND.
THE EE WAS PACKING 50# BAGS. THE BAGS WENT ON TO A CONVEYOR BELT WHEN THE BELT STOPPED, THE PACKED CLIMBED ONTO THE BELT WITHOUT LOCKING IT OUT TO STRAIGHTEN OUT THE BAG. HE TRIPPED THE ELECTR ICAL EYE AND THE BELT STARTED. HIS FOOT WAS CAUGHT BETWEEN TEH END OF ONE BELT AND THE START OF THE OTHER BELT.
THE EE WHO WAS A PACKER WAS WATCHING THE BAG TO MAKE SURE THAT THE CODE CHANGED AT MIDNIGHT. HE STEPPED BACK IN TO THE LANE FOR THE FORKLIFT AS THE HYSTER CAME AROUND THE CORNER. HYSTER TIRE H IT HIS ANKLE AND HE FELL HITTING HIS KNEE.
