While trouble shooting a mechanical issue, EE went into the collector ring area of the 191M shovel. EE did not follow company policy for Lock out/ Tag out. During the trouble shooting EE got too close or touched the collector ring and received an electrical shock to the head area.

Helping another employee remove a 1/2 sheet of plywood from under a belt. When he picked it up to pull it out he heard a pop and felt a pain in his left lower leg. Pulled a muscle or possible tear to MCL or Achilles rupture.
Removed shive from pump,placed on ground , shive toppeled over striking left foot of employee. Left foot had swelling, brusing and a small contusion. Employee was taken for x-rays. There were no broked bones. Employee was return to lite duty work.
Inflating small tire. Tire was wrapped by a ratchet strap. Went to release strap. The strap released with great force striking left hand. Small cuts to pointer and ring finger. Stitches were required. The miner returned to work that same day.
Annual Audiogram was performed and revealed a threshold shift of 13.3 in his left ear.*** Second test found no reportable hearing loss***
While changing v-belt on crusher feeder drive come-a-long slipped off anchor point and struck victim on right shoulder.

Injured employee was working with 4 others to replace teeth on cutter head. Employee placed his hand on the tug push knee to look over side to check teeth for damage. Tug shifted and employee's left hand was caught between cutter head teeth and push knee. Employee attempted to pull hand back and left ring finger nail was pulled away and tip of finger broken inside glove.
The employe was struck below the right knee, causing an open tibia fracture, by a piece of metal banding that was released from under a front-end loader as the employee was spotting the front end loader operator.

EE WAS GREASING A GRADER (V-JOINT) IN A CONFINED SPACE USING A COMPRESSED-AIR GREASE GUN. EE WAS HIT IN THE EYE BY GREASE WHICH CAME THROUGH THE NIPPLE AND SHOT BACK ONTO HIM. HE WAS WEARING SAFETY GLASSES BUT THE GREASE STILL MANAGED TO GET IN HIS EYE.
EMPLOYEE WAS CHANGING SCREENS ON THE TOP DECK OF THE #4 SHAKER. HE WAS USING A PRY BAR TO ADJUST THE SCREENS WHEN IT SLIPPED SMASHING HIS FINGER BETWEEN THE PRY BAR AND THE SCREEN.
EE WAS REMOVING THE FRONT BORE OF A 30" PIPE WITH AN IMPACT GUN WHEN THE PIPE MOVED AROUND TOWARDS HIM CRUSHING HIS HAND AGAINST THE WALL.
EE WAS WASHING OFF AROUND TAILPULLEY OF A PLANT STACKER WHEN A ROCK BOUNCED OFF OF STACKER STRIKING HIS FOOT.

EE WAS GRINDING A PIECE OF METAL WHEN THE GRINDER KICKED BACK & CUT THE MEDIAL LT HAND.
