EE entered feeder when gate would not close. While cutting out a piece of metal that was blocking the slide gate, material sloughed off and engulfed the EE.

Employee was welding a bucket on an excavator when a piece of slag broke off and rolled down their company uniform onto their under shirt causing it to catch fire resulting in second degree burns to their left upper arm.

Employee was changing out bottom conveyor rollers. Employee was placing the bad bottom roller into the work truck when EE's middle right finger was lacerated causing injury.

EE was assisting with changing the hoist ropes when their left index finger became pinched between the tugger rope and hoist drum. This resulted in a fracture with a laceration to the tip of the left index finger.
Employee was rolling up a spray hose mounted on the rear of the water truck. The employee over reeled the hose causing the hose nozzle to strike employee's lip causing a laceration.
EE was working on tower three changing out punch hole metal screens. The EE was making final positioning adjustments and smashed left thumb between the existing and new punch hole metal screen causing laceration.
Employee was removing pins from a quick-connect device on a 966 loader when the quick-connect device rolled from the cribbing onto the employee's left foot. The employee experienced fractures to the large toe and the second toe along with a laceration to the large toe. Employee was wearing safety toed boots.

Employee was driving an A40G haul truck with the seat belt on in a parked position when employee was rear-ended by a 45G haul truck traveling approximately 8MPH. Impact caused employee's body to shift forward and left shoulder hit the steering wheel resulting in a cervical strain.
EE was carrying a 45 lb roller when EE lost control of the object causing it to fall, crushing EE's right index finger between the roller and the concrete. The finger was not broken but a prescription was given for swelling.
EE was trimming palm trees when a palm frond sprung back and a thorn punctured EE's hand. First aid was performed but swelling occurred. Triage doctor referred EE to the clinic and antibiotics were prescribed. No further treatment was given.
Injured employee (IE) was in the process of disconnecting the cam lock for a grout hose. As the IE was uncoupling the hose, the remaining grout in the hose, made contact in the facial area. After the IE removed their safety glasses, IE felt irritation in EE's Left eye. IE's eye was flushed onsite and IE was taken to the hospital for evaluation.

EE was on the floating conveyor to move the conveyor chute car. A come-a-long was attached to the car to help move it to the correct place. The EE was brushing some rocks off of the track when the car moved and smashed EE's finger tip.
EE was driving the man-lift and hit a washout causing the man-lift basket to bounce. The EE struck the handrail with their chest/side.
Employee was cutting rubber while on knees and fell back while attempting to shift weight and accidentally cut themselves with the knife in the leg causing injury.

Crew was unclogging the crusher when one of the large rocks came out of the rigging and struck the handrail, breaking it. The crew installed barricade tape and continued working. The IW was working on the platform and backed through the barricade tape and fell to the ground. Coworkers asked if EE was okay, EE responded that EE was fine and went back to work.
Employee was cutting belt with knife and came in contact with hand causing injury.
EE was tightening the plug on the gear box when the wrench slipped causing EE's left forearm to get caught on a piece of metal catwalk material.
Tech was using a wire wheel, a bristle came loose and penetrated the skin in the thigh area on the left leg. Tech went to the hospital to have the piece removed.
Clamp failed holding counter weight to belt sled. Weight fell causing the belt to move while they were on it and employee was bounced back into the top belt.
Injured EE and another EE were replacing a return roller on the carry back conveyor when the roller shifted. The injured EE moved quickly to hold the roller and felt a pain in side/back.

While trouble shooting a mechanical issue, EE went into the collector ring area of the 191M shovel. EE did not follow company policy for Lock out/ Tag out. During the trouble shooting EE got too close or touched the collector ring and received an electrical shock to the head area.

While walking on a catwalk employee stepped on a rock and twisted his left ankle. Employee was taken to hospital and diagnosed with a sprained ankle; also referred to a bone and joint specialist for further treatment. Employee is unable to work for three days or until specialist clears him.
Loader operator hit a pot hole in travel way to load a truck. The operator said that he missed filling in the pothole during his early morning Inspection.
