Showing all 11Employee resigned on 07/14/2017. Company received mailed notice on 9/13/2017 that employee is claiming cumulative injury to both shoulders and upper extremities. The exact nature and cause of alleged injury is unknown. No report of injury was made prior to resignation.
Employee was shrink-wrapping a pallet of bags by pulling a shrink-wrap roll around the pallet. As employee bent down to wrap the bottom part of the pallet, he felt a pain in his lower back. No excessive pushing or pulling occurred, injury occurred while bending over. Employee was diagnosed with a lumbosacral back strain.
