Employee was using pry bar to open inspection door on preheater tower. Pry bar slipped striking employee in knee. (Did not become reportable until employee started to miss work due to surgery on 2/17/10.)

While collecting cement samples, employee slipped on ice falling to the ground. It was still snowing at the time of the accident.(Did not become reportable until he started to miss time due to surgery on 3/3/11.)

Develo0ped pain and numbness in both hands from operating 75 ton haul truck. Did not become reportable until he had carpal tunnel surgery on 5/13/11.

Employee was using shovel and jackhammer to remove hardened buildup inside the clinker cooler when he developed pain in his shoulder. (Did not become reportable until he started to miss work due to surgery on 3/31/11.)

While using an air lance to remove kiln feed build up in preheater tower, employee tripped over support beam. (Did not become reportable until he started to miss work due to surgery on 2/24/11.)

While pulling old dust collector bags off cages in main baghouse, employee felt strain in his shoulder. (Did not become reportable until employee started to miss work on 1/10/2011 due to surgery).

While working on various pieces of production equipment, employee felt pain and tingling in his hand. (Did not become reportable until he had carpal tunnel surgery on 11/19/10.)

Employee developed pain and tingling in hand and fingers after using shovel and jackhammer. (Did not become reportable until he had carpal tunnel surgey on 11/12/10.)

Employee was welding on guard rail when his auto-darkening welding lens failed. He immediately replaced lens with a standard welding lens.

Employee was installing chain fall in a choken when the chain fall slipped out of his hand striking his finger. (Reportable due to stitches.)

While carrying tools up steps, employee twisted knee resulting in sharp pain. (Did not become reportable until employee started missing work due to surgery on 3/25/10.)

As employee was responding to fire alarm, he slipped on concrete floor, striking his chin on desk. (Stitches required.)

Supervisory employee walked a short distance into the kiln (system shutdown and locked out) to determine if temperature had dropped sufficiently to allow brick peeler to remove built up coating. As he did so, not realizing that there was coating overhead, it fell striking him in the back and knocking him down.

While working to replace roll in coal mill, employee noticed pain and tingling in his left hand. (Did not become reportable until he had carpal tunnel surgery on 1/27/10.)

Employee was welding on finish mill shell (overhead and in awkward position). Felt pain in shoulder. (Did not become reportable until he started to miss work due to surgery on November 16, 2009.)

While fabricating an access platform, employee stepped on a piece of angle iron causing him to twist his right knee. (Did not become reportable until September 28, 2009 when he started to miss work due to surgery.)

Employee was kneeling while reconnecting wiring on mill bearing temperature detector. As he stood up, he felt a pop in his knee. (Did not become reportable until employee started to miss work due to surgery on 10/5/09.)

As employee was removing guard from apron pan conveyor, the guard caught, causing him to lose his balance and take a couple of steps backwards. As he did so, he stepped in a pool of hot kiln feed.

Employee was climbing ladder on loader when he slipped. He held on with his right arm to prevent fall, resulting in strain to shoulder. (Did not become reportable until employee started to miss work due to surgery on 7/13/09).

Injured employee and co-worker were carrying part of an airslide up a small set of stairs, when he felt pain in his knee.(This injury did not became reportable until EE started to miss work on May 4, 2009 due to surgery Misc Mail 5/8/09 #2)

As employee was investigating plugged temperature probe on preheater tower, hot cement puffed out of port entering employee's eyes (was wearing protective clothing including face shield).

Employee and coworker were carrying a pallet jack downstairs when he felt a strain in his back. (Was not reportable until he had surgery on 3-26-09. Was on layoff status at the time.)

Employee was operating chain fall when he felt a popping sensation in his shoulder. (Did not become reportable until he had surgery on 4-20-09)

Injured and coworker were moving an empty Con-Ex shipping container with two forklifts. The forklift the injured employee was operating started to tip and employee unfastened seat belt and attempted to jump free resulting in injury to his leg.

Employee was performing routine maintenance on vacuum truck, when a large hose fell from an elevated position, striking him in his left chest. (Did not seek medical treatment until 102408.)

Side of dozer slipped when employee passed crest of stockpile causing him to be partially ejected from seat. Seat belt came unhooked (Seat belt was not defective, but was apparently not securely fastened.)

Employee hooked up air hose to cement silo cone and turned on air. As he was inserting safety clip into coupling, he heard air leak. As he was adjusting the coupling, it came apart, striking him in the mouth.

While moving concrete blocks from flood wall to bed of truck, employee strained his back. (Did not start missing work until 7/24/08)

Employee was climbing a stationary vendor ladder attached to equipment. He was grasping for the next rung and his hand slipped and he fell backwards 4'. His back hit against an adjacent railing.

While moving mill grinding ball hopper into position, the ball on the hopper rolled over, pinching the employees hand between handle and ball. (Required stitches)

EMPLOYEE FELL OFF SCAFFOLDING.

As employee entered new cooler, he stumbled catching foot between rollers.

Three workers were handling several lengths of pipe. Pipe slipped, pinching middle finger of left hand of one of the workers, breaking the finger just below the fingernail.

Employee was using steel bar to remove hardened materials when he felt a strain in his shoulder. (Did not become reportable until he started to miss work due to surgery on 5-08-08.)

As plate was being removed from junction box, the cover slipped, pinching employees finger. (Stitcfhes required).

Employee was climbing onto loader when his foot slipped, causing him to strain arm and shoulder. (Did not become reportable until he started missing work due to surgery on 2-29-08.)

Dismantling elevated boom section about 5 feet in the air, while stepping off of boom section he got foot caught in lanyard. Uneven contact with ground resulted in twisting of right ankle.

Injured was removing E-stone pan feeder guard to repair it, when the guard slipping and a broken piece of metal cut his finger. (Received 4 sutures)

Employee was standing on a 6' step ladder when the pipe wrench he was using slipped causing him to lose his balance and fall to the floor. (Was less than 5' above floor level.)

Employee & his crew were disassembling a light plant above ground. Buildup of ice on roof of auxiliary building broke loose & fell approximately 30-40 feet striking EE in the head & neck, knocking him to the ground. He has two herniated disks in his neck per physician & MRI study.

Injured was removing sign from access platform when he felt burning pain in his arm. Did not become reportable until he started to miss work due to surgery on 12-06-07.

As employee was walking around crane to string cable, he stepped on clinker, causing him to twist his knee. (Did not become reportable until he started to miss work on 2/11/08 due to surgery.)

A bolt, connecting a roller guide and mounting plate to the top of the temporary construction personnel hoist, broke. The EE was struck with a glancing blow to the head and left shoulder and resulted in a fracture to the EE's left upper arm.

EE was setting a 6 foot concrete column form and when the form started to tip over, he attempted to keep it from falling and he stepped off the concrete footing and fell approximately 3 feet to the ground below and struck his left knee and right ankle on rocks and concrete rubble, which resulted in a torn meniscus in his left knee and a sprain to his right ankle.

EE was in the laydown yard, cleaning trash and moving railroad ties. EE alledged he hurt his back while moving ties by hand. EE did not seek medical help for 2 weeks. EE's doctor recommended days away from work starting 10/25/2007. EE's doctor advised he had a possible herniated disk in lower back.
