Supervisory employee walked a short distance into the kiln (system shutdown and locked out) to determine if temperature had dropped sufficiently to allow brick peeler to remove built up coating. As he did so, not realizing that there was coating overhead, it fell striking him in the back and knocking him down.

Employee & his crew were disassembling a light plant above ground. Buildup of ice on roof of auxiliary building broke loose & fell approximately 30-40 feet striking EE in the head & neck, knocking him to the ground. He has two herniated disks in his neck per physician & MRI study.

A bolt, connecting a roller guide and mounting plate to the top of the temporary construction personnel hoist, broke. The EE was struck with a glancing blow to the head and left shoulder and resulted in a fracture to the EE's left upper arm.

EE was setting a 6 foot concrete column form and when the form started to tip over, he attempted to keep it from falling and he stepped off the concrete footing and fell approximately 3 feet to the ground below and struck his left knee and right ankle on rocks and concrete rubble, which resulted in a torn meniscus in his left knee and a sprain to his right ankle.

EE's doctor recommended days away from work starting 10/25/2007. EE's doctor advised he had a possible herniated disk in lower back.
