Haul truck driver was hauling material to the asphalt plant, on EE's return to get another load there were two customer trucks blocking the haul road. EE parked EE's haul truck and asked the two drivers to move and not block the road. Another customer truck struck EE, knocking EE to the ground, resulting a laceration to EE's left arm and a head injury.

Showing all 8Removing a bracket that had a handrail attached to it, sitting the bracket on the ground and the handrail struck the EE teeth.
Will was exiting cab and lost footing on the access ladder. Will fell approximately 55" to the ground. It resulted in non-displaced posterior malleolar and oblique fractures through the distal right tibial metaphysis. A minimally displaced oblique fracture of the proximal right fibula.
EE was accessing crusher off the catwalk next to the crusher. EE stepped up approximately 6" above the side of catwalk onto the crusher frame. Right foot slipped off causing the right foot to (wedge) between the frame of the crusher and the catwalk frame. This action caused the right foot to slip/twist sideways causing the right ankle to break.
HE HAD SURGERY TODAY. CARPAL TUNEL SYNDROME.
EE VISITED THE DR ON 3-30-93 HE HAD SURGERY ON 4-7-93.
BREAKING UP CLODS OF MATERIAL WHEN PRY BAR SLIPPED, MASHING LEFT INDEX FINGER BETWEEN BAR AND SAND BINS.
THE DEE WAS ATTEMPTING TO ADJUST A CONVEYOR BELT WHEN HE SLIPPED & HIS ARM WAS CAUGHT IN THE TAIL PULLEY. HE LOST HIS LEFT ARM COMPLETELY AT THE SHOULDER.
