EE had an umbilical hernia prior to employment with Rolling Rock. EE has no idea what EE did to aggravate it. Hernia is larger and needs repair. Due to aggravation and enlargement it is work related and does not fall under short term disability.

EE advised us letter dated 8/4/14, was received by employer on 8/8/14, illness to lower back is related to wear & tear from 13 year tenure at Greystone Quarryes, Inc. Attached copy of letter for reference. E-mail, ee advised us, as per his doctors communication and the x-ray results, the correct date for the injury/illness is 6/17/14. EE was recently diagnosed w/carpal tunnel syndrome 6/20/14 & 6/26/14. Carpal tunnel syndrome be a gradual occurrence. To have a specific onset date is almost impossible to say. EE gave us an approx. date of 4/21/14 we were notified by ee of this on 6/16/14 in a casual conversation.
EE was walking past a co-worker when the co-worker decided to toss a piece of waste stone into a waste hopper which contacted EE on top lower R-forearm causing laceration requiring multiple sutures. EE medical follow up requires surgery on tendon scheduled for 07/02/2013.

EE was pushing & positioning a rock on a roller conveyor into the head of a hydraulic stone splitter when EE heard a popping sound in L-shoulder. L-shoulder felt sore. EE did go to his doctor on occasion with no restrictions or missed work days however, upon advice of doctor EE just completed surgery on L-shoulder on 9/13/13 & will be out for undetermined time during rehab.

Picking a building stone from a hopper the top part of the stone broke free, employee lost his grip and the broken piece fell on left forearm causing a cut.
After completing a cut with hydraulic stone splitter employee pushed cut stone thru cutting head to off side conveyor & caught left forearm on sharp edge of stone. Equipment did not directly cause injury. Injury required couple sutures
Pulling stone on roller feed conveyor attached to hydraulic stone splitter the employee shoulder started hurting. When moving shoulder pop could be heard. Possible sprain (r) shoulder. Stone was being pulled as normal operation on roller feed tray of splitter. At time of incident was not reported to upper management. Reported on 7/12/11. Off work effective 7/12/11.

Employee was hand trimming & packing stone at #3 saw conveyor at thin stone bldg. Employee experienced pain in right elbow running down to middle two fingers. Injury was originally reported on 8/17/10 & employee continued to work until 2/28/11. At this time due to current Dr's restrictions, employee is not working and is awaiting elbow surgery scheduled on 3/17/11.

EE reported that he nailed 2 pieces of wood together w/nail gun. He laid down the gun on his right side & proceeded to pick up a piece of wood when he claims the gun fired. He claims he was not touching the gun & it was 18" away when it misfired & shot him through the top of left hand going completely through but remaining exposed on both sides. Accident under investigation.

A LARGE STONE FELL OFF THE SIDE OF A ROLLER CONVEYOR WHICH FEEDS HYDRAULIC STONE SPLITTER. EE TRIED TO MOVE THE ROCK WITH A CROW BAR WHEN HE PULLED A MUSCLE IN RIGHT CHEST AREA. HE SHOULD HAVE WAITED FOR FORKLIFT TO MOVE STONE.
