Caught in, under or between a moving and a stationary object

While installing the lower chisel head on a splitter using a prybar to locate the lower head bolt into place the bar slipped and the head shifted smashing the employees left index finger requiring 6 stitches. Employee is on limited duty with left hand until stiches are removed on 3/16/2026.

Struck by flying object

While trimming stone with a masons hammer a piece of the hammer head chipped off and embedded into the inside of the employees left wrist. The Dr says it will need to be removed and surgery is scheduled on 10/7/2025. Employee is allowed to work until then with restrictions

Caught in, under or between a moving and a stationary object

While lifting up a stone tread off of a stack of treads to see the bottom side, the tread slipped and fell onto the employees left middle finger pinching it between two treads. Employee suffered a smashed open fracture/crushing injury to the finger with nail damage.

Struck by flying object

While driving in a new pin on an excavator track a piece broke off and entered the top of EE left forearm. EE went to the Dr. and said to have it removed surgically. EE missed 1 day of work, the day of surgery. Dr. let EE come back to work the day after surgery to be able to work at a level comfortable for EE. No other restrictions were given.

Struck by flying object

While grinding an Excavator Bucket for repairs, a spark went under employee's safety glasses into EE's left eye. Dr didn't find anything in eye just prescribed a prescription antibiotic eye drop.

While moving a large stone on a conveyor belt EE smashed right index finger between the stone and the conveyor frame. Laceration on top and bottom of finger with swelling. Urgent Care cleaned and inspected wound for any broken bones & none were found. Returned to work with no restrictions. EE was completely discharged from their care after a 2 day follow up.

Over-exertion in wielding or throwing objects

EE Was in the Wood Shop making palletizing Boxes using a pneumatic nail gun. EE felt discomfort in right shoulder at the end of shift. EE reported the discomfort the following day when the discomfort didn't seem to get better. EE was then sent occupational health care where the Dr. described it as Strain of Muscle and Tendons of the right shoulder Rotator Cuff.

Employee felt discomfort in the top of right forearm while sorting and repacking stone into crates. The pain developed over time and was not one specific incident. The Dr. says there is a strain of the extensor muscle, Facia and Tendon at the Forearm level of the Right Arm.

EE was in Building #18 operating Saw #4. While loading a larger stone on the feed conveyer they tried pushing the stone up against the stone in front of it on the conveyor belt. EE then heard a pop in left chest and felt some pain. The Doctor says they fractured a rib on the left side.

EE was sorting stone and was carrying a stone back to the splitter for a recut. EE stumbled and fell into machine with stone in hand, EE's finger was smashed between stone and machine. EE's post Drug and Alcohol test came back inconclusive and sent for further testing. Test came back 5 days later Positive. EE was Terminated due to Positive Drug and Alcohol Test.

Employee was using a folding utility knife to trim a piece of rubber window molding when knife slipped and cut the top of EE's left pinky finger resulting in 9 external sutures.

While pulling stone scrap pieces from underneath picking conveyor with a push broom EE felt discomfort in right shoulder. This is one of the job tasks associated with assisting in the stone sorting operations along with lifting and placing stones from conveyor into pans. EE could not pin point the exact moment injury occurred. Pain had worsened over time.

EE woke up 11/19/20 with pain in neck & worked all day. EE reported issue to mgt. 11/23/20. EE did not recall any specific incident causing pain. EE was pushing/pulling slabs on gravity roller conveyor leading to hyd. splitter. EE indicated EE did nothing at home to cause injury. Dr. report 11/23/20 indicates sprain of ligaments & strain of other muscles @ left shoulder upper arm.

Struck by falling object

While packaging stone EE attempted to hand a co-worker a rock which slipped out of EE's hand and landed on EE's left foot directly behind steel toe. X-ray of Left foot revealed a fracture at base of Left 5th toe. EE did not seek medical attention until pain and swelling developed on morning of 11/4/20.

EE had an umbilical hernia prior to employment with Rolling Rock. EE has no idea what EE did to aggravate it. Hernia is larger and needs repair. Due to aggravation and enlargement it is work related and does not fall under short term disability.

EE was bending a piece of steel which was inserted into a vice. During the process of bending the steel, it slipped out of the vice and snapped back into EE's face.

EE was trying to move a stone into a loader bucket. All was ground level. No lifting involved. When ee gave a last push - ee felt a pop in lower back. EE sprained back.

Repetitive movement when handling stones. CTS - Carpal Tunnel Syndrome. Employee was on light duty since the date of injury until surgery date. Absence started effective 9/1/16.

Cutting, hammering & chiseling on sidewalk to place a stone ramp. When trying to stand up straight, lower back popped. Injury was not caused by a tool or another person, it was simply getting up from a low position.

Sweeping floor when employee felt a sharp burning pain in right groin area. NOTE: Due to annual company shutdown which stats 12/24/15 to 1/3/16 - work resulted 1/4/16, Jan 4th 2016 became the 8th day for reporting this injury which includes the date of injury.

Employee was hand trimming & packing stone at #3 saw conveyor at thin stone bldg. Employee experienced pain in right elbow running down to middle two fingers. Injury was originally reported on 8/17/10 & employee continued to work until 2/28/11. At this time due to current Dr's restrictions, employee is not working and is awaiting elbow surgery scheduled on 3/17/11.

EE reported that he nailed 2 pieces of wood together w/nail gun. He laid down the gun on his right side & proceeded to pick up a piece of wood when he claims the gun fired. He claims he was not touching the gun & it was 18"" away when it misfired & shot him through the top of left hand going completely through but remaining exposed on both sides. Accident under investigation.

EE came into office at end of the day complaining about being hot and said that he needed a break. EE arrived at work on 06/11/08 and reported not feeling well, after some observation we decided to call for an ambulance.
